Job Title: Wellness Counselor/Educator, Chickasaw Nation Wellness Centers
Job Summary:
- This position is located within the Chickasaw Nation with the duty station being at the following: Ardmore, Tishomingo, Ada, or Purcell Wellness Centers
- The purpose of this position is to develop, implement and coordinate activities for the purpose of maintaining and improving the wellbeing of participants in our service population.
- The position will encourage exercise and proper nutrition by providing on-site and on-going training services to participants.
- A positive outlook on life will also be promoted by encouraging participants to develop their physical and mental potential.
- This position may require days, evenings and weekend hours. Supervision Received: Incumbent is under the direct supervision of the Wellness Center Manager. Routine work is performed independently following written procedures and standards and regulations.
Job Functions:
1. Performance Management:
- a. Plans, develops and implements health/wellness activities according to Identified needs and desires of participants and health care providers.
- b. Provides guidance and educational counseling services to individuals, families And groups on health concerns including but not limited to diabetes, cardiovascular problems, nutrition management, exercise, safety, etc.
- c. Plans, develops, implements and /or coordinates wellness screening activities with other tribal and non-tribal entities, i.e. health fairs, blood cholesterol checks, blood pressure, fitness assessments, etc.
- d. Creates opportunities for individuals, groups and families to participate in organized exercise programs within the Chickasaw Nation, i.e. aerobics, walking, weight training, etc.
- e. Maintains inventory, equipment and supplies for the program. Checks out equipment for program activities and ensures its return to the department.
- f. Upon referral from CNDH providers, provides assessments and develops treatment plans for individual patients. This may include one on one counseling, education, instruction and information and referral services.
- g. Maintains records of participation in health/wellness activities and prepares a report to document the program’s progress and performance.
- h. Participate in continuing education opportunities as required.
2. Organizational Management:
- a. Assist other departments in providing training/seminars, and/or educational Opportunities for individuals, families or groups which focus on specific health concerns of that department, i.e. diabetic management, eldercare, nutrition, etc.
- b. Treat all members with respect and maintain a cooperative attitude with other employees to facilitate quality patient care.
- c. Maintain effective working relationship with employees and supervisory staff of the facility, with tribal officials and other visitors to the facility.
- d. Serve as a member of any group or committee concerned with health/wellness development within the Chickasaw Nation.
- e. Perform public relation assignments as directed by the Ardmore Wellness Center Manager.
3. Perform other duties as assigned.
MINIMUM QUALIFICATIONS:
Knowledge, Skills, and Abilities:
- Must have the ability to understand and follow written and verbal instruction, communicate effectively, both orally and in writing, with the public and other health care providers.
- Must have the ability to perform work independently with minimal supervision.
- Demonstrate ability to maintain in strict confidence all information received on the job.
- Must possess a minimal understanding of special populations including sensitivity to the Native American culture.
- Must project a professional image at all times.
- Demonstrate community organization, creative planning and program development skills with a high level of analytical ability to disseminate and coordinate program services within the existing structure without interrupting Chickasaw Nation operations.
Education and Experience:
- A minimum of a Bachelor’s degree in a health related field is desired.
- One year experience in providing direct care services within a health care organization is desired with experience and flexibility in service provision with all age groups.
Licenses and Certifications:
- A valid Oklahoma State driver’s license is required.
- Must possess or be eligible to receive certification in CPR/First Aid, water aerobics, aerobics, and other areas identified within the program.
