Job Posting for Home Visiting Family Support Specialist - Morgan and Camden Counties at The Community Partnership
Job Summary:
The Home Visiting Family Support Specialist is responsible for maintaining regular contact with families, primarily through home visits, and providing referrals to other needed services. PLEASE NOTE: This position will work from their home, however, family-focused support will be provided through in-person home visits in Morgan and Camden counties.
Duties and Responsibilities:
Schedule and conduct visits with enrolled families according to the frequency and standards of Healthy Families America.
Follow through on referrals made to the Home Visiting program by other agencies and professionals.
Establish family goals and maintain records of achievement for each family.
Maintain accurate, timely records of all contacts, services and materials delivered to families and record in the required database.
Qualifications
Bachelor’s degree in human services or fields related to working with children and families from an accredited college or university is preferred
Individual must have excellent written and verbal communication skills, be computer literate, out-going, creative, organized, and resourceful, a self-starter, and professional.
