What are the responsibilities and job description for the Discharge Planner Hospital Case Manager BSW MSW position at THE CONNECTICUT HOSPICE, INC?
Primary Job Focus: Clinical social work assessment, case management, discharge planning, intervention, and consultation to patients, families, interdisciplinary staff, and the community on issues related to Hospice care, anticipatory grief, bereavement, death, and dying. Provide support, teaching, crisis intervention, discharge planning, referrals to appropriate community resources and other duties as assigned. May be responsible for supervising Social Work Interns.
Reports to the Director of Social Work
Education: MSW preferred, BSW considered, from an accredited school.
Experience and Qualifications:
- Integrity, reliability, maturity and professional boundaries.
- Experience working with a terminally ill or geriatric population.
- Two years of Case Management and discharge planning experience in a similar setting.
- Strong knowledge base of relevant community resources entitlement programs.
