This is a hybrid teleworking position, with the expectation that you will be working at our office location in Millersville, MD at least once a week.

What You Will Do
The Care Coordination Associate (CCA) is an administrative position that is responsible for supporting the Rare and Expensive Case Management Program (REM) by coordinating, implementing, and administering specific activities related to all aspects of The Center’s care management policies, procedures and programs. You will work alongside licensed care coordinators who work with a caseload of clients with complex medical needs and ensure they have all the services and supports to remain independent in their community setting of choice.
As a Care Coordination Associate, you will:
- Provide primary clerical support including processing memos, reports, letters, consents, and correspondence, as well as a variety of other duties including photocopying, scanning, e-faxing, mailing, organizing/processing documents, and electronic filing.?
- Verify active Medicaid insurance for all assigned clients on a monthly basis and update the electronic database with any changes.
- Support our Clinical Care Coordinators with tasks including processing applications and forms, durable medical equipment (DME) and pharmacy requests, locating funding sources, and other special projects as needed.
- Request any necessary medical record information from providers and clients to streamline communication amongst the client team.
- Triage calls and other inquiries from clients/families, providers, and outside agencies and address any issues or concerns with assigned coworkers.
- Serve as Administrator for a council/committee which includes scheduling meetings, taking meeting minutes, and assist facilitator with other duties as needed.
- Update different electronic database systems including Outlook, updating client information and caseload changes.

Salary : $32,800 - $41,500
