Overview:
We are seeking a dedicated Care Coordinator to join our team. Responsible for interviews, hires, evaluates and disciplines resident services staff with Director of Nursing. Establishes time schedules for resident services staff on specific shifts. Assigns and delegates responsibilities to resident services staff for the specific residents based on the qualifications of the staff and needs of the residents. Ensures each resident receives services prescribed or requested in accordance with federal and state regulations and facility policies and procedures. Assures prompt, appropriate action in cases of injury, illness, fire or other emergencies. In addition, serves as Marketing Ambassador in the ongoing effort to increase and/or maintain 100% census at all times.
Duties:
- Coordinate and oversee care plans for residents, ensuring individual needs are met
- Monitor and document resident progress, behaviors, and vital signs
- Assist with activities of daily living (ADLs) and provide direct support as needed
- Uphold resident rights and promote independence and dignity
- Collaborate with healthcare professionals to optimize resident care
- Implement memory care strategies for residents with cognitive impairments
Requirements:
- Experience in senior care, nursing homes, or working with individuals with developmental disabilities
- Proficiency in ADLs, vital signs monitoring, and first aid
- Knowledge of systems for managing resident care effectively
- Strong interpersonal skills to work with residents, families, and healthcare providers
- Ability to adapt to changing environments and prioritize tasks efficiently
