Title: Independent Housing Assistance Case Manager
Schedule: Monday – Friday 8am to 5pm
Location: Longview and Vancouver
Classification: Full-time non-exempt
Opening Date: May 11, 2022
Salary Range: $21.00 - $23.00 hr./ $43,680 - $47,840 annual
Closing Date: May 24, 2022
Position Summary:
Responsible for providing case management and advocacy for Native American families experiencing homelessness towards self-sufficiency. The primary goal is to address the barriers families face regarding permanent housing through culturally relevant and a strength-based process.

Essential Job Duties and Responsibilities include the following, and other related duties as assigned.
Knowledge, Skills, and Abilities:
- Provides information and outreach to all CIT Programs/Departments to identify homeless Native Americans for referrals to the Program. Coordinate with Tribal and community programs to identify homeless Native Americans.
- Conducts outreach and develops partnerships in our communities to locate housing options for families. Establish landlord relationships. Develop relationships with community homeless shelters and other necessary resources for collaboration and partnerships.
- Works collaboratively with clients to develop a Family Goal Plan using strength-based and culturally appropriate approach to short and long-term goals.
- Provides referrals for assessments and evaluations to determine appropriate services from other CIT programs/services, local, state and federal resources. Participates in Coordination and Care meetings.
- Participates as a team member including staff meetings and sharing clients when appropriate, with Vocational Rehabilitation (VR) and Cowlitz Career and employment Services (CESS) sharing knowledge, expertise and caseloads. Using a team approach maximizes client employment opportunities, economic self-sufficiency and independence while minimizing cost and resources.
- Completes monthly home visits supporting families with housing stability, including timely payment of rent and household maintenance. Encourages families to take part in financial planning.
- Perform case management duties maintaining current and accurate files and update information as required. Maintains a data system of client information (Rite-Track) and records in the national Homeless Management Information System (HMIS).
- Upon approval, purchases services and goods for clients as needed, maintaining fiscal guidelines. Work closely with office staff with purchase orders etc.
- Attends trainings/classes/workshops and other technical assistance available to learn best practice and new information for clients. Travel will be required.
- Collaborate with schools/counselors for outreach and referrals, to aide in educational success for homeless students.
- Maintains client statistics and submits activity reports. Assist with conducting audits for effectiveness and compliance. Participates in Peer Review Activities. Participates in database trainings.
- Informs and consults frequently with the VR Program Manager regarding program activities, issues, concerns and milestones.
- Travels to meet with clients outside of the office, which may include home visits or public places and may include transporting clients.
- Honor and represent the mission, vision and values of the Cowlitz Tribe Health and Human Services.
- Possess strong organizational skills and work efficiently independently as well as in a team setting.
- Have a strong understanding of the Native American population, including historical trauma and cultural appropriateness.
- Must have experience working with or personal knowledge of the plights of homeless families including mental health, substance abuse and other barriers that reflect an understanding of homelessness.
- Other duties as assigned.
Additional Requirements:
- Maintains client confidentiality, trained in HIPAA and Ethics.
- Telephone and computer skills with knowledge of general office equipment.
- Professional work ethics, manner and appearance.
- Ability to communicate orally and in writing.
- Works cooperatively within office, tribe and other agencies as needed.
- Must be punctual and dependable in regular attendance.
- Analytical Requirements: Assignments frequently involve decisions based on a wide knowledge of many factors where application of advanced or technical concepts are predominantly used.
- Ability to interact with people with diverse social, cultural, economic and educational backgrounds.
- Ability to make public presentations and participate in outreach activities for public events providing information for the program and other Cowlitz Tribe HHS services.
Required Education:
High school diploma or equivalent
Supervisory Responsibilities: This position has no supervisory responsibilities.
Requirements:
This position requires qualified applicants to be fully vaccinated including COVID-19 vaccines on their first day of employment.
Must submit to and pass a criminal background check and drug screen.
