Location: Based in the State of Kansas, with our PulsePoint Office located at 1010 SW Tyler St, Topeka, KS 66612. Travel within the state will be required as necessary.
Our ideal candidates will live within a reasonable proximity to our Elevance Health major office (PulsePoint) located in Topeka, KS. Elevance Health supports a hybrid workplace model with PulsePoint sites used for collaboration, community, and connection.
The Manager I GBD Special Programs - LTSS is responsible for managing and overseeing a team responsible for coordination of Healthy Blue’s comprehensive health care program in which Kansas long-term services and supports (LTSS) members' needs are identified, including physical health, behavioral health, and social services for older adults, LTSS populations and services, and Home and Community Based Services (HCBS) coordination.
How You Will Make an Impact
Primary duties may include, but are not limited to:
Manages resource utilization to ensure appropriate delivery of care to members, adequate coverage for all tasks and job responsibilities.
Coordinates service delivery to include member assessment of physical and psychological factors.
Participates in cross-functional workgroups created to maintain and develop program.
Evaluates current processes of Special Program's support functions; recommends changes for increased efficiencies and improved outcomes.
Develops and conducts training programs for staff involved in the program.
Extracts and manipulates analytical data to present findings to relevant markets and stakeholders.
Hires, trains, coaches, counsels, and evaluates performance of direct reports.
Minimum Requirements:
Requires a BA/BS and minimum of 5 years’ experience in a related field, including minimum of 1 year leadership/management experience; or any combination of education and experience, which would provide an equivalent background.
Preferred Skills, Capabilities, and Experiences:
Service delivery coordination, discharge planning or behavioral health experience in a managed care setting preferred.
RN, LSW, LPC, or LPN/LVN license in the State of Kansas is strongly preferred.
Case and Care Management experience is strongly preferred.
Experience with KanCare HCBS Waiver programs strongly preferred
