What are the responsibilities and job description for the Independent Living Counselor position at The Forum Group?
We are seeking a dedicated and compassionate Independent Living Specialists to come
Qualifications:
- Must have a GED or HS diploma
- Must be familiar with CARES and AWARDS
- Must have prior experience with the MICA population
Duties
- Provide comprehensive case management services to a challenging caseload of mentally ill, homeless adults.
- Assist with obtaining benefits, referrals for treatment, monitoring hygiene and teaching independent living skills, escorts to and advocacy for entitlements and housing appointments, and housing placement.
- Facilitate weekly psycho educational group.
- Generate ongoing progress and group notes as well as monthly statistical reports. Participate in clinical team, case conference, and other staff meetings.
- Provide emergency first aid/CPR assistance when needed.
- Perform additional related duties as assigned
