The Guidance Center (TGC) is currently seeking Client Care Navigator to be part of our innovative interdisciplinary team. TGC is a Community Mental Health Center serving Leavenworth, Atchison, and Jefferson counties in Kansas. Responsibilities
- Work with clients and families to gather and complete NOMS questionnaires at grant intake and for six-month follow-up/discharge data collections points
- Submit NOMS data for SPARS entry and/or enter NOMS into SPARS system in a timely manner
- Assure that patient information is communicated to key staff prior to and after appointments
- Serve as single point of contact for grant enrollees who require care coordination
Qualifications
- High School diploma or GED
- Experience working with Electronic Health Records to provide prompt electronic documentation of clinical services.
