The Program Director/QIDP position with California MENTOR/ Loyd’s Liberty Homes supports the individuals we serve in our Intermediate Care Facilities (ICF), which is a residential-based service line. We require an experienced leader who can successfully direct program staff to provide high quality services to individuals we support. In this role you will manage the cases of 12 individuals and the day-to-day operations for two group homes with the support of your Area Director, Quality Assurance and Human Resources team.

Job Responsibilities:
- Work with treatment team to develop and implement individual Service Plans (ISP); conducts intake assessments as applicable
- Maintains regular interaction with direct support team and individuals served to ensure on-going assessment and follow-through of treatment plans; review service plans regularly and attend annual and ongoing meetings
- Case management includes coordinating services and medical and/or therapeutic treatment with consultation from medical and therapeutic professionals, as well as licensing agencies and family members, etc.
- Provides on-call coverage for caseload including after-hours emergency and crisis intervention support
- Financial responsibility in the following areas: operational budget, time-keeping and billing for services provided
- Prepare incident reports for both internal and external agencies; participate in internal investigations and follow up/related action as required
- Directs and supervises assigned staff in day-to-day operations; includes performance evaluations, scheduling, orientation, and providing regular training; staffing homes through interviews/hires staff, and other changes to employee status
- Performs other related duties and activities as required

What we require of you:
- Bachelor’s degree in a human services related field
- At least 18 months experience in the management of a human services delivery system
- –OR– At least five years of experience in a human services delivery system INCLUDING at least two years in a management or supervisory position
- Prior experience as an administrator for group homes, or as a QIDP (or QMRP), a BIG PLUS, but not required
- Valid CA Driver's License
- Access to a registered vehicle with proof of insurance – travel as needed
- Must pass background investigation that includes reference checks and FBI/DOJ live scan, as well as a physical examination and TB test
- Emergency on-call availability night/overnight and weekends required – these homes and the individuals we support require 24/7 care so this position does require 24/7 availability by phone
