Scope of Position
The Social Worker for Survivorship works collaboratively in the development, planning, implementation and evaluation of supportive and psychosocial services and programs geared to adult cancer patients and their families. This position provides psychosocial support and case management interventions for cancer survivors and caregivers within Survivorship, Psychosocial Oncology Counseling, and Palliative Harm Reduction clinics.
Position Summary
The position will be responsible for managing counseling referrals to the clinic to ensure parity and access for cancer survivors. The Social Worker will perform the initial screening assessment to determine eligibility, mental health acuity and triage appropriate care and provide support to patients on the wait list. The Social worker responsibilities also include case management, crisis intervention, financial counseling, health education and the provision of material resources and linkage with community agencies specifically for the James survivorship clinic including support of Psychosocial Oncology services.
This position will support the multidisciplinary Palliative Harm Reduction clinic that provides pain management services to patients undergoing cancer treatment who also have an active substance use disorder. The Social Worker is a direct patient care position addressing aspects of substance dependence utilizing a patient centered harm-reduction approach. The social worker will engage in screening and initial assessment of substance use disorders, and provision of individual alcohol and other drug interventions, harm reduction, and health promotion interventions; these are patient-centered harm-reduction interventions to help initiate and maintain positive behavior change, and to empower patients with the information necessary to make informed decisions about their health and well-being. The social worker will provide support to patients diagnosed with cancer who also have an active substance use disorder and/or complex psychiatric diagnoses requiring increased case management and psychosocial interventions. The social worker will provide direct patient psychosocial assessment, case management and interventions, outreach and follow-up calls, connection to internal and community resources, and assist with discharge planning that may include connection to community Suboxone/ Subutex and mental health/psychiatric services.
Core competencies include: Psychosocial assessment, counseling and crisis intervention skills, Knowledge of the community alcohol and drug resources, and skills assessing substance abuse.
Master of Social Work from an accredited Graduate School of Social Work; State of Ohio Social Work licensure (LSW or LISW) or equivalent education/experience.
