Job Title: Social Worker Department: University Hospital | Social Work and Case Management Administration Care Facilitation: 70% of time Conduct psychosocial assessments, evaluation, counseling and interventions for patients and their families to assist them in coping with: Medical diagnosis resulting in acute and chronic illness Disability resulting from the diagnosis or pre-existing disability Terminal condition Death Violence and maltreatment including domestic assault, child and elder abuse/neglect Substance abuse Relationship problems Mental Health problems Environmental problems including financial, housing, transportation, and activities of daily living. Vocational/educational concerns. Facilitates discharge planning coordination including transitions to Skilled Nursing and Long Term Care facilities, Long term acute care(LTAC), inpatient rehabilitation, and referral to mental health agencies/hospitals. Facilitate Advance Directives and assist with durable power of attorney and guardianship procedures. Forms a working relationship with patients and families to educate regarding disease specific needs, levels of care, and available resources Provides emotional support to patients and families while assisting with decision making and encouraging compliance with plan of care. May provide transplant evaluations Demonstrates an understanding of the impact of life threatening illness or injury on patients of all ages, and their caregivers and significant others. Proactively communicates with treatment team members (physicians, nurses, Clinical Case Managers, UM Case Managers, Rehab team, financial services) to provide updates related to the discharge plan, potential barriers to discharge, and readmission risks. Delegate appropriate tasks to Program Coordinator/Extendor and Office Associate as needed Appropriately utilizes development funds and community resources to support patient need Partners with Utilization and Clinical Case Managers to identify and present outlier cases at Outlier Care Management meetings Serve as consultant to other professionals on health care team on psychosocial issues and community resources Quality: 25 % of time Participates in process improvement, task forces, committees, and projects supporting Department and Enterprise Strategic goals. Enters quality concerns in Event Report system Stays current with evidenced based practice, literature, research, and Best Practice Guidelines related to care management Demonstrates awareness of readmission risks and works with treatment team to identify/implement interventions to reduce risk Maintains an awareness and knowledge of Medicare Conditions of Participation related to discharge planning; joint commission regulations; Social Work standards of practice.
