The Department of Pediatrics at the University of Virginia’s School of Medicine seeks applicants for a Community Resource Coordinator to support Blue Ridge Care Connection for Children. This position is responsible for providing case management through direct care coordination/case management service to children with special health care needs. This position will provide assistance with social and practical environmental problems to Blue Ridge Care Connection for Children's patients and their families as outlined by the grants. It is responsible for providing emotional support to pediatric patients and their families who may exhibit feelings of anxiety and stress precipitated by illness and hospitalization. This position works with patients and team members in an effort to maximize patients' potential for the healthiest coping responses to their challenging situations as outlined in the grant's scope of work. In accordance with grant requirements, the care coordinator/case manager will consent the patients, abstract and enter data, and with the research guidelines. Based on overall assessment and summary of patients and family needs, makes appropriate community referrals as needed. Serves as a liaison between the medical staff and the patient/family and provides interventions designed to minimize patient stress and maximize coping skills to expedite recovery.
The successful candidate will be expected to:
Create, implement, and assess new and innovative health promotion programs.
Maintain collaborative networks between all peer education programs.
Conduct focus groups, intercept interviews, and utilize other data sources to identify prevention needs and create programs to address those needs.
Design and conduct qualitative and quantitative program assessment to ensure research-based programming.
Facilitate event logistics including event communication, production of event support materials, and archiving materials upon event conclusion.
Promote health promotion activities and services through media outlets. Ensure website content is current and updated, as needed.
Participate in continuing education and other professional development activities. Attend conferences and professional development activities.
In addition to the above job responsibilities, other duties may be assigned.

Qualified candidates must have:
Education: Bachelor's degree in higher education administration, health promotion, counseling, or related field of study.
Experience: At least four years of experience in professional field.
Preferred Qualifications:
Bilingual (English/Spanish)
Familiarity or experience working with social services or public health education; knowledge of local support services and community organization
Experience in public health education, social work, medical systems navigation, and/or community health
Required knowledge, skills, abilities:
Professional and empathetic interpersonal skills to assist culturally diverse individuals during times of distress
Positive attitude, strong work ethic, and sound judgement
Excellent written and oral communication skills
Highly organized and proficient in data management
Proficiency with Google Suite (Gmail, Drive, forms, etc.), Excel, and Redcap (or similar survey/database software)
Demonstrated ability to work independently and coordinate with a variety of stakeholders

This is primarily a sedentary job involving extensive use of desktop computers. The job does occasionally require traveling some distance to attend meetings, and programs.

Anticipated Hiring Range: $45,000 to $65,000 commensurate with education and experience.
