Minority Health/Health Equity Liasion
Job Specific Responsibilities:
Minority Health and Health Equity: Work to address health disparities and advance health equity in Calhoun and Liberty Counties. Ensures requirements from the Office of Minority Health and Health Equity (OMHHE) are complete, timely and compliant with regulatory and funding requirements. Participates in outreach activities designed to improve health and enhance the quality of life for Calhoun and Liberty County residents. Designs, reviews and analyzes activities and data with position supervisors to aid in planning, resource utilization and provision of services. Supports community development processes and ensures deliverables related to the process are met (including but not limited to participating in routine conference calls). Works with clinical and program staff to assist with reducing racial and health disparities in Calhoun and Liberty County. Works with public health information officers and operations personnel to ensure culturally competent, diverse and inclusive marketing, programming, and services. Advocates and promotes for public health services and programming to Calhoun and Liberty County residents, partners and visitors alike. Supports health promotion, public health education, and health equity activities through development of & participation in workshops, seminars, school presentations, task force meetings, youth advocacy board meetings, health screenings, etc. Attends trainings as it relates to Health Equity, chronic diseases, and community health improvement. Correctly completes inputs and submits timesheets and EARs. Ensures budgets and OCAs related to minority health and health equity are complete, timely and compliant with regulatory and funding requirements.
This is a highly responsible professional position with lead responsibility for coordinating COVID-19 health disparities intervention activities in Calhoun and Liberty County. The primary responsibilities of this position include mobilizing partners to advance heath equity and address social determinants of health as they relate to COVID-19 health disparities among populations at higher risk and that are underserved. This position is responsible for building community capacity that includes traditional public health organizations and non-traditional partners to reach disproportionately affected populations with tailored programs and practices for testing, contact tracing, vaccination, and health care strategies.
Convene and facilitate a multi-sector health equity coalition that includes members of underserved communities and organizations that serve the community. Engage partners in building/updating COVID-19 plans and health equity plans with the intention of establishing systems that can also support future emergency responses.
Build cross-sectional partnerships to align public health, healthcare and non-health (e.g., housing, social service) interventions that decrease risk for COVID-19. Support county activities and outreach to increase access to COVID-19 prevention, testing and vaccination to populations that are high-risk and underserved.
Identify and establish collaborations with critical partners affiliated with and who provide services to populations that are underserved and at higher risk for COVID-19 to disseminate accurate, culturally and linguistically responsive information and facilitate access to health-related services. Work with local faith-based, education, health and social service agencies and other community gatekeepers to increase participation in community outreach, data collection and emergency planning.
Assess training needs of county health department staff and other community partners to improve their capacity for Knowledge Coordinate training activities with the Regional Health Equity Coordinators assigned to CHD.
Timely and adequately complete reporting of county COVID Health Disparities work plan and budget. Attend required COVID Health Disparities trainings.
Community Partnerships: Responsible for developing partnerships within the community in order to avoid duplication of health services. Assure the best use of limited resources for disease prevention and health promotion in the community as it relates to racial and ethnic health disparities. Coordinates and provides other state priority program activities for residents of Calhoun and Liberty County.
Accreditation: Actively engages in and supports the goals of Calhoun and Liberty’s Community Health Assessment and Improvement processes (including but not limited to attending meetings and participating in activities). Works with Quality Improvement leadership to develop evaluation tools and methods of examining effectiveness to ensure departmental and/or programmatic accountability. Provides updated information on the health status of Calhoun and Liberty County regarding racial and ethnic health disparities. Ability to use and present data from the Florida Health Charts system with staff and public. Attends trainings related to quality improvement.
Health Education: Supports Health Education programs and initiatives such as Tobacco Prevention, Heart Health, SNAP-ED, Healthiest Weight, Healthy Moms and Babies among others to improve service delivery, the quality of programming, and overall customer satisfaction.
Required Knowledge, Skills, and Abilities:
Knowledge of minority health and health equity concerns within underserved , high risk populations. Skilled in using the Microsoft platform to include Outlook, TEAMS, Word and Excel. Highly skilled in verbal and written communication. Ability to work independently. Ability to coordinate and lead outreach efforts. Ability to work cohesively with community partners. Ability to follow a written programmatic plan outlining expectations and deliverables.
Qualifications:
Valid Driver’s License. High School Diploma or Equivalent.
