A day in the life of an MDS Coordinator / Expectations:
- Works in collaboration with the care team to assess the needs of the resident.
- Monitors MDS and care plan documentation for all residents.
- Completes accurate coding of the MDS with information obtained via medical record review as well as observations and interview with facility staff, resident, and family members.
- Monitor and ensure care plans are adhered to.
- Properly documented residents receive the best care.
- Complies with State and federal Regulations.
- Works collaboratively with members of the healthcare team.

Qualifications
Qualifications:
- Valid State license(s) RN (required)
- Three (3) or more years MDS experience (preferred)
- Point Click Care experience (preferred)
- Must be a self-starter and able to work independently and efficiently
- Commitment to team-oriented outcomes and quality care
- Excellent written and verbal communication skills
- Reliable transportation
- Proof of Covid-19 vaccination or exemption
