What are the responsibilities and job description for the Intensive Transition Care Manager position at The Village For Families & Children?
Starting salary $42,000 will commensurate with education/experience.
The Intensive Transition Care Manager (ITCM) will be responsible for facilitating a timely and successful discharge from an acute or subacute level of care and will remain with the child and his/her family while in the community to ensure the successful transition from acute care to the community. The ITCM will be responsible for connecting the child to the clinically appropriate outpatient services, including extended day treatment and intensive outpatient treatment.
DUTIES & RESPONSIBILITIES:
- Maintains a case load of 10 to 12 families which will include including weekly face to face contact with the youth and the family: serving a minimum of 80 families
- Assembles a Child and Family Team meeting within two weeks of enrollment by interviewing the family and identifying family members/natural supports, school/agency representatives and other significant persons
- Assesses the child's/family's strengths and needs and promote strategy development with assistance from team members
- Coordinates the Child and Family Team meetings and ensure that they are held at least every 30 days to review progress
- Develops and completes the initial Plan of Care in the first 2 weeks based on the family's strengths and needs, including a comprehensive twenty-four-hour Safety/Crisis Plan; Subsequent Plans will be updated as needed every 30 days after the initial Plan
- Monitors, with the Child and Family Team, the provision and quality of services as recommended by the Child and Family Team
- Assist the family in assessing appropriate mental and behavioral health services, educational services and other appropriate supports identified by the Child and Family Team
- Ensure that the services being provided are community based and are responsive to the family's vision and needs identified in the Plan of Care
- The length of service for ITCMS for each child and his/her family will be 8-12 weeks with the possibility of an extension for additional time in the program.
- The child and his/her family will be seen weekly maintain a case load of 10 to 12 families which will include including weekly face to face contact with the youth and the family.
- Completes all required documentation and statistical reports related to client service in accordance with agency procedure
Required Experience:
Education/Experience:
Master’s Degree in Human services and experience working with youth with behavioral health issues and their families preferred, Bachelor’s Degree in Human Services with at least 5 years of experience working with youth who have behavioral health issues and their families required.
Knowledge and Skills:
