Summary/Objective:
The Voyage Senior Living employs Registered Nurses and Licensed Practical Nurses at The Anchor, our memory care facility. The nurses assume professional responsibility for a wide range of resident care and treatment, telephone triage and other duties in support of the memory care facility environment in keeping with his/her training and experience. The RN and LPN are professionally supervised by the Nurse Manager and the Administrator. The RN and LPN functions as a member of the team supporting all medical, emotional, nutritional, educational, ethical and social services provided at the memory care facility and participates in quality improvement activities.

· Demonstrates proficiency using the nursing process, including but not limited to, evaluations, assessments, and implementations. Escorts resident to apartment, performs complete clinical assessment including vital signs, records objective complaints and findings with timely notification of primary care provider.
· Prepares residents for examination/treatment and assists with diagnostic procedures and treatments when indicated.
· Secures health and social histories from residents and/or family members and records findings in a systematic and accurate manner utilizing compliance with HIPPA legislation.
· Charts pertinent findings and information in resident’s medical records and assists in maintaining medical records including filing of all reports, correspondence, and material pertinent to the resident’s care; maintains logs, tickler files, and other systems to aid in continuity of care and retrieval of resident data.
· Recognizes significant factors in history and assessment, utilizes critical thinking skills in distinguishing abnormal versus normal findings, and consults with physicians or practitioners as appropriate.
· Recognizes and manages specific problems/conditions in keeping with established medical protocols, standing orders, and medical care policies and procedures of The Voyage Senior Living, seeks professional guidance as appropriate when protocol is not clear or does not exist, works with appropriate management to develop needed protocol and procedures.
· Administers medications to residents as prescribed by physician and performs testing and screening procedures, injections and immunizations for Memory Care Facility residents as ordered by the physician or midlevel practitioner.
· Performs basic lab procedures including venipuncture and prepares specimens as needed.
· Schedules medical referrals, tests, and treatments ordered by physicians or midlevel practitioners and assures that resident is notified. If unable to understand instructions, will contact family members with information and assists with completing referral.
· Maintains a working knowledge of community and area resources and refers residents to resources as appropriate.
· Performs infection control procedures and sterile technique and regularly applies this knowledge to prevent transmission of disease.
· Complies with all standard operating procedures of the Memory Facility regarding employee health and safety, including but not limited to OSHA regulations (Bloodborne Pathogen Standard and Hazardous Communications Rules), EPA regulations governing waste disposal, (MSDS) sheets and use of appropriate personal protective equipment and completion of incident reports.
· Performs inventory of medications (including samples) at regular intervals, logs all medications received, e.g.: injectables, inhalers/nebulizers, ointments, syrups, and verifies/records all scheduled narcotics administered to residents ensuring strict adherence to pharmacy policies and procedures.
· Assures that all medical supplies and equipment are maintained in an appropriate manner, including cleaning and sterilizing of instruments, speculums, etc., insures current expiration date, and that all equipment is in good operating condition; reports any problems, malfunctions to administration; orders necessary supplies and equipment and maintains an adequate supply at all times.
· May be required to collect and record data, and prepare reports for Administrator and Nurse Manager and IDPH for funding needs.
· Participates in staff meetings and committee meetings as assigned.
· May be temporarily assigned to work at other The Voyage facilities when illness or staff shortages exist.
· Conducts self in a professional, helpful manner at all times; maintains confidentiality of all matters.
· Collects behavioral data, coordinates with Administrator/Nurse Manager regarding behavioral assessments, and implements behavior intervention plans with proactive and environmental strategies.
· Shares on-call duties with other staff as requested by the Nurse Manager or Administrator
· Performs all other duties as assigned

Physical Demands and Expectations:
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
· The employee is regularly required to talk, hear and have the ability to follow written and oral instructions and procedures
· The position requires frequent sitting, walking and standing, occasional bending, stooping, kneeling, twisting, stretching/reaching and climbing stairs. Requires occasional lifting and carrying up to 15 pounds.
· Must have dependable transportation, a valid Driver’s License and State required liability insurance on a personal vehicle
· May be required to work a flexible schedule and be willing to come in and assist workers to ensure that residents’ needs are met
· Must be willing to attend in-service training, seminars and other activities as assigned
· Appropriate documentation indicating absence of infectious/communicable diseases, e.g. TB and Hepatitis may be required
· Must be able to apply nursing skills in direct resident care and have knowledge of disease processes.
· Must have current Healthcare Provider Basic Life support/CPR card
· Must be able to function as a member of the Memory Care Facility team
· Position is subject to a criminal background check
Education and Experience:
· Graduate of an accredited school of nursing. Licensed to practice as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of Illinois.
