GENERAL DESCRIPTION
This position is responsible for the delivery of varied social work services to Home Health Care clients and their families. Purpose of visit is to provide initial psychosocial evaluations, ongoing psychosocial counseling, direct casework services, and community education, outreach and referral. The social worker is an integral part of The Whole Person’s Home Health Care group.
ESSENTIAL JOB FUNCTIONS
- Prepares a thorough psychosocial history and Plan of Care to guide the home health care team in meeting the social and emotional needs of caring for the client
- Participates actively in the meetings and facilitates discussion of issues relating to the psychosocial component of care.
- Assists the client and his/her family to utilize existing resources in the home environment or refers client and family to community agencies and acts as an intermediary for client as necessary.
- Assesses, when appropriate, a family financial situation, taking into consideration the client’s prognosis and medical need, then facilitates the referral process to any agency for financial assistance.
- Provides counseling services to the client/family experiencing emotional, social economic, or other issues.
- Assists in discharge planning from home health care service when care is no longer appropriate.
- Participates in orientation and in-service training programs for professional staff.
- Records clinical/progress notes and documents on the Plan of Care in the client’s medical record.
- Assists clients and their families with personal and environmental difficulties, which predispose them to illness and interfere with their obtaining maximum benefits from medical care.
- Documents all client-related activities in a timely manner.
- Reacts to change productively and performs other job-related tasks and duties as assigned.
REQUIRED SKILLS
- Must have a Degree in Social Work.
- Experience with Home Health Care

WORK ENVIRONMENT & PHYSICAL DEMANDS
May be exposed to extremes of heat and cold weather conditions. Must be able to travel during various inclement weather conditions.
May be exposed to infections and contagious disease. Contact with participants under wide variety of circumstances. May be exposed to participant elements. Subject to varying and unpredictable situations.
