Compensation: $58 per visit which includes mileage
GENERAL DESCRIPTION
The licensed Speech Pathologist will provide Speech Language Pathology (SLP) in compliance with the physician’s Plan of Care.
ESSENTIAL JOB FUNCTIONS
1.Evaluate the patient’s needs, including medical, psychosocial and environmental needs. Complete comprehensive OASIS Initial Assessment and opening paperwork if primary discipline.
2.Develop and implement, with physician and patient, individualized Plan of Care based on patient’s physical, medical, psychosocial and environmental needs, and make appropriate revisions as the patient’s condition changes. Establishes appropriate realistic measurable goals for patient to achieve, then evaluates progress towards these goals.
3.Complete comprehensive OSASIS Reassessment/Follow-up Form, if Speech Therapy is only discipline ordered, if there is a significant change in the patient’s health status.
4.Complete comprehensive OSASIS Discharge Assessment if last skilled discipline providing service.
5.Complete comprehensive OSASIS Recertification/Follow-up, if Speech Therapy is only discipline ordered, along with other recertification paperwork as appropriate.
6.Provide testing and treatment program to improve speech, language, motor and swallowing abilities.
7.Observe and report patient’s progress to the physician.
8.Instruct other TWP staff, when necessary, and coordinate care of services with other involved organizations and individuals.
9. Refer patient to a facility when more extensive diagnostic evaluation is required.
10.Educate patient and/or family regarding the therapy program and safety. Prepare patient and/or family for discharge.
11.Document, accurately and timely, the patient’s treatment, care, evaluation, and progress, and communication with other staff members.
12.Participate in in-services, quality improvement activities, patient care conferences, workshops and seminars. Assume responsibility for continuing education and maintaining current knowledge.
REQUIRED SKILLS
- Graduate from Speech-Language Pathology Program
- Current license as a Speech-Language Pathologist in the State of Missouri
- Minimum one (1) year experience in a health care setting
- Must meet Clinical Competency requirements
- Ability to travel to patient’s homes within designated geographic area
- Valid driver’s license and proof of auto insurance
- Current CPR Certification

WORK ENVIRONMENT & PHYSICAL DEMANDS
This position but also will travel throughout a designated geographical location to visit patients in their homes. While performing the duties of this job the employee has the potential to encounter unpredictable clients. The position has mostly an inside work environment and occasional outside work, traveling from one location to another, with exposure to weather conditions.
