Summary :
The RN Patient Flow Coordinator leads the daily operational functions that integrate patient access, patient placement, and staffing resources. Coordinates and collaborates with leaders to strategically allocate resources across ThedaCare to optimize clinician throughput against patient throughput to improve responsiveness to patient needs. Requires strong skills in communication, customer service, multi-tasking and real-time problem solving. Contributes to the goals of the department by being accountable for the delivery of compassionate and safe care within the scope of practice as defined by the Wisconsin Board of Nursing and ThedaCare policy. Through collaborative practice with members of the care team, is responsible for patient outcomes that meet the high quality of care provided by ThedaCare. Furthers the professional practice of nursing at ThedaCare by promoting a culture of innovation and a commitment to growth and professional development.t.Job Description:
SCHEDULE:
12 hour NIGHT shift (5:30p-6a)
Every 3rd weekend rotation required
Rotating holiday schedule required
KEY ACCOUNTABILITIES:
Collaborates with multiple departments to effectively and efficiently support inpatient units to provide safe, quality patient care and achieve the desired patient outcomes.
Serves as a liaison between physicians, department leaders, team members, and internal and external customers.
Ensures accurate and timely transfer of information to support patient placement and necessary staffing resources.
Proactively anticipates priority for admissions, transfers, and discharges.
Anticipates patient flow, oversees and coordinates bed assignments from all points of entry, and works in conjunction with physician and/or team members to prioritize admissions based on medical need.
Answers and routes phone calls, pages physicians for appropriate transfer, ensures physician approval to provide care for incoming patients, and provides oversight of efficient placement of patients and alignment of team member resources.
Develops and supports effective team interactions through open and honest communication, receptive to feedback, mentoring, and developing others.
Develops and supports relationships that create an environment where individuals and team members learn and grow professionally.
Accurately tracks and documents information and metrics assigned using current technology.
Identifies and participates in process improvement and problem solving in the work to decrease defects/barriers, enhance customer satisfaction, and reduce waste and/or cost.
Resolves problems by anticipating and intervening in areas of customer need, providing timely and accurate corrections to defects.
Communicates changes efficiently and effectively with impacted parties.
Understands and supports the change management process on the organizational and departmental levels.
QUALIFICATIONS:
Current Wisconsin RN Licensure
Bachelor's of Science in Nursing (BSN) OR Associates Degree in Nursing with an agreement to obtain the BSN within five years.
American Heart Association Healthcare Provider Basic Life Support (BLS)
PHYSICAL DEMANDS:
Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of Fifty (50) pounds without assistance
Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties
Manual dexterity and hand-eye coordination to perform patient care procedures
