What are the responsibilities and job description for the Hospice Chaplain position at THEMA Health Services?
- Responsible for the provision of spiritual care services to patients/families/caregivers, either directly or through coordination of care with other spiritual counselors.
- Requires the ability to determine the level of care and support of bereaving families and significant others following the death of a Hospice patient, re-evaluating the family/significant others bereavement needs for a minimum of 12 months.
- The chaplain will coordinate and implement the plan of care provisions related to the bereavement/spiritual needs of families/significant others, which will include(but not limited to) visits, cards, and letters, organizing support groups, organizing one-on-one and individual counseling, memorial services, and other assistance programs that may be developed.
- Adhere to company policies and procedures and regulatory compliance requirements and directives.
Experience:
1. Bachelor’s degree in Social Work or Counseling or equivalent theological degree from a college or university, or equivalent experience.
2. Experience with patients, families, and caregivers dealing with death and dying, grieving, and/or dealing with loss.
3. Knowledge and commitment to hospice philosophy.
4. Ability to establish therapeutic relationships with members of diverse lifestyles, cultures, faiths, beliefs, and values.
5. Ability to work within an interdisciplinary setting.
6. Self-directed with the ability to work with little supervision.
