The ideal candidate will be results-driven, team-and detail-oriented professional who is comfortable working on multiple tasks in a deadline-oriented environment.
Responsibilities: To provide specialized services to clients who are chemically dependent and/or their significant others. Provide and coordinate substance abuse assessment, education, training, and placement services for clients of Community Support Services programs.
- Conduct initial substance abuse assessment services to all clients on team.
- Provide individual, group, and family substance abuse treatment and counseling.
- Coordinates referrals to other substance abuse treatment programs or facilities in the community/state, i.e. medically assisted treatment or inpatient.
- Provide chemical dependency consultation, education and training to team personnel.
- Develop and implement treatment plans and document services consistent with Center standards.
- Coordinate discharge planning, follow up and aftercare.
- Monitor signs and symptoms of psychiatric functioning; medication regimen, and substance use.
- Assist in community networking and education.
- Provide assistance with income maintenance, living arrangements.
- Provide education, training and modeling in basic daily living skills, social or parenting skills.
- Teach clients effective techniques for managing their medication as prescribed.
- Coordinate services within the Center as needed.
- Assist clients in developing and strengthening their support network to accomplish goals and objectives.
- Complete billing forms, logs, data collection forms and clinical documentation in accordance with Center standards.
Requirements:
- Bachelor Degree in behavioral health field or equivalent combination of education and experience required.
- Two (2) years of relevant substance abuse experience.
- CDP or working toward credentialing. LCDP preferred. Valid driver’s license required.
