A Hospice (RN) Case Manager is needed in Roswell, GA.
Titan Placement Group is currently seeking a compassionate and experienced Hospice RN Case Manager to join a reputable healthcare team in Roswell, GA. Salary and Benefits:
- Competitive Salary: Starting at $80,000 - $90,000
- Health Insurance: Employer covers 75%

- Mileage Reimbursement: $0.50 per mile

Responsibilities:
- Monday – Friday
- Provide hospice care services to patients
- Conduct home visits to assess patient needs and provide care
- Visit 3 - 5 patients per day
- Collaborate with interdisciplinary healthcare team members
Requirements:
- Clear and active RN license in the state of Georgia
- Preferred experience in Hospice, Home Health, ICU, CCU, Oncology, End-of-Life Care, or MedSurg
- Compassionate and dedicated approach to patient care
