TLC HomeCare is looking for 13 accomplished Caregivers, PCAs, HHA, and LNAs to work in our clients' homes throughout Upper Valley Vermont and New Hampshire; Windsor and Grafton County areas. We have immediate openings for Full-Time Days, Evenings, and Night Shifts. We have both Part-Time as well as Per Diem openings. Unlike other home care agencies, TLC guarantees a Full-time position, which means guaranteed hours and a stable paycheck. To expedite your onboarding, we are conducting Zoom interviews and Zoom Day 1 of our orientation.

Caregiver/PCA
Position Description:
Personal Care Assistants or Caregivers are unlicensed members of the TLC HomeCare team who assist clients with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs). Caregivers are expected to perform as outlined in a written care plan approved by a Nurse Case Manager. The Care Plan is available to caregivers in the client's home and online through TLC. All caregivers are supervised by a nurse manager. All caregivers receive scheduling direction from the HomeCare Coordinator. Occasionally caregivers serve clients living in a facility setting. The principles of this job description do not change as a result of various client home environments.
Qualifications:
1. Must be at least 18 years of age.
2. Must be able to read, write, speak and understand English.
3. Will preferably have a minimum of one year of work experience as a caregiver which is verifiable through references, and must complete the agency’s employment process successfully.
4. Must be physically and cognitively able to perform the duties of the position, including documentation tasks.
5. Must be able to understand the need for and principles of confidentiality.
6. Must be able and willing to follow and/or learn care directions.
7. Must be able to accurately report observations and facts to nurse managers.
8. Must have available, reliable transportation to and from assignments.
9. Must be willing to work every other weekend and holidays as needed.
10. Must be willing to travel at-least 30 miles from your home to provide care for others.
Duties:
The duties of a caregiver may include, but are not limited to the following:
1. Planning and preparing meals according to specific dietary requirements of the client.
2. Maintaining a safe, clean and healthy environment through light housekeeping, including changing bed linens, dusting and vacuuming, cleaning kitchen and bathroom, and laundry.
3. Providing companionship and stimulation for the client including reading, walks, etc
4. Accompanying the client to doctors or other appointments.
5. Grocery shopping or other errands with or without the client as needed.
6. Performing other delegated tasks as indicated in the Care plan.
7. Perform any personal care for the client, including toileting, hair care, oral hygiene, bathing and grooming.
8. Assist the client with position transfers and other types of mobility as directed by the Care Plan
9. Monitor the client’s self-administration of medications and/or use of medical devices as specified in the Care Plan
Caregivers May Not:
1. Administer medication unless specifically delegated by a TLC Nurse Case Manager
2. Cut the fingernails or toenails
3. May not take responsibility for any financial tasks of a client, e.g. writing checks or going to the bank for the client.
4. Perform any care task beyond the scope of this job description that is not delegated for a specific client by a nurse manager
Guidelines
• Read and follow the Care Plan and instructions for care at the start of your each shift with each client. Then, review the Caregiver Service Binder in the home when it is used at the beginning of each additional shift.
• Document properly all you do for the client. This may be in the form of a task check-off log or other written documentation which is found in the Caregiver Service Binder. Remember to sign your name to such documentation.
• Call the office to speak with the nurse on-call or a case manager before performing any functions for which you feel inadequately trained.
• IMMEDIATELY notify the office of any change in your client’s condition. The nurse on-call or a case manager will contact you.
• IMMEDIATELY notify the office if your client or you fall or are injured. Follow the emergency protocol described by TLC policy and procedures. Complete an incident report as requested by TLC management.
• Death is usually expected, and specific issues regarding care of the client during and after death have been addressed ahead of time. If you have any questions concerning your client, call the office. If death occurs suddenly, follow instructions for “Emergency Situations” described in TLC policy and procedures.
• ALWAYS CALL the office, if you have any questions about or inability to complete your job responsibilities.
