Hope Cooperative is searching for a Housing Case Manger .

Description: Hope ACTs Housing Case Manager has the responsibility of working closely with Hope ACT staff and residents in associated housing, who are progressing toward independent living. The Housing Case Manager provides onsite support needed for each client resident to function at the highest possible level. These supports will include emotional support, demonstrating and teaching living skills, coordinating program activities, facilitating groups, and client activities. The Housing Case Manager also has the responsibility of completing tenant records and related occupancy documentation required by the Agency, Rental Assistance Programs, Leased Housing, SHRA and HUD (per related HUD guidelines) for Hope ACT Properties and clients. In addition, this position is responsible for processing paperwork to verify that the applicant/resident meets regulatory set-aside requirements of the properties, maintaining waiting lists and working with the Program Managers and Personal Services Coordinators in qualifying and processing applicants to fill vacant units.

This position is a full-time, non-exempt position and may require working some evening and weekend schedules.

ESSENTIAL DUTIES and RESPONSIBILITIES:

  • Support individuals with active listening, demonstration of empathy and carry a message of hope and recovery.
  • Observe, assess, intervene and refer (within one’s scope of practice) in situations with clients regarding mental health symptoms, physical health symptoms and substance use issues.
  • Develop service/goal plans with clients and meet with the clients as needed to encourage, support and advocate for progress and assist with skill development.
  • Provide/Assist the client with transportation in personal vehicle as necessary.
  • Provide rehabilitative services to clients in the community and their homes.
  • Provide positive public relations, community referrals, information and support by linking clients to needed resources; advocate with and on behalf of clients and significant support persons the clients have identified.
  • Accurately complete all necessary forms, reports and paperwork required by the program on a timely basis (e.g., progress notes, special incident reports, discharge summaries, Medi-Cal billing, Avatar forms, HMIS).
  • Facilitate planning of social, cultural, educational, supportive and recreational group activities.
  • Shop for food and household supplies to meet program or client needs.
  • Ensure the office, house, rooms, units and yard are clean and fit for occupancy, where applicable. Provide and assist with cleaning as needed.
  • Assist with CAHI, HUD and SHRA Annual File Reviews and HQS Inspections.
  • Prepare documentation and related applicant and client occupancy and annual paperwork.
  • Follow and enforce all Fair Housing and local Landlord and Tenant laws and regulations.
  • Assist the Director of Program Coordinator to ensure that the property and yard are clean and fit, meeting California's State Licensing Standards Section 800887: "The facility shall be clean, safe, sanitary, and in good repair at all times for the safety and well-being of clients, employees, and visitors".
  • Collect, record, and deposit payments for Agency Program fees received from tenants and clients. Post rents, HAP payments and fees to Agency Property Management software.
  • Enter move-in and move-out information to Agency software and process Security Deposit Refund paperwork. Complete Agency documentation on client residential status (lease, move in/outs, room changes, income changes, etc.)
  • Ensure that up-to-date tenant files are maintained per HUD standards.
  • Ensure the protection and confidentiality of all Protected Health Information (PHI) by following all policies and procedures in the Agency HIPAA Manual.


Education and/or Experience:

  • Ability to work with individuals who have a mental health diagnosis. Experience is highly desirable.
  • Lived experience as a mental health consumer or a family member/loved one of a mental health consumer is desirable.
  • Mental health crisis intervention experience is highly desirable.
  • Must have work experience with persons who have substance abuse problems, preferably individuals who are dually diagnosed (mental health & substance abuse).
  • Must have knowledge of 12-Step Programs and/or personal experience with substance abuse recovery.
  • Mental health crisis intervention experience is highly desirable.
  • Knowledge of housing vouchers and Yolo housing is highly desirable.


Certificates, Licenses, Registrations:

  • Provide own vehicle, possess a valid California Driver’s License and current vehicle insurance. Driving record must meet and continue to meet the generalized driving guidelines used by our insurance broker.
  • Must have at least 3 years of driving experience.
  • Must obtain CPR and First Aid certificates within the first six (6) months of employment.


Other Skills, Abilities, and Job Requirements:

  • Ability to pass clearance of the Federal Bureau of Investigation (FBI) and/or Department of Justice (DOJ) background screenings.
  • Consumer of mental health services or a family member of a mental health consumer is desirable.
