Position Description:
Provide skill building and daily living supports to individuals in the performance of daily and independent living skills within their home and community as outlined in the home and community-based services policies and procedures.
- · Plan, perform, or assist with care or activities with individuals according to their preference or needs, to include, but not limited to, household chores (cooking, cleaning, personal hygiene, laundry, dishes, vacuuming, fixing bed, household maintenance, etc.), community involvement, social, recreation and leisure activities, communication, personal safety, and behavior.
- · Provide opportunities for new experiences in the community or home.
- · Focus on the individual rather than the disability.
- · Follow, implement, and provide feedback with person-centered and behavioral outcomes.
- · Maintain professional boundaries between team members, families and individuals served.
- · Provide guidance and direction while being a positive role model to the individual.
- · Assist in problem-solving issues.
- · Be creative by trying new ideas that create a more positive impact for the individuals receiving services.
- · Provide transportation to recreational activities, doctor’s appointments, and employment; etc. and/or support individuals with acquiring transportation as needed.
- · Look for ways to promote and improve the quality of A Village Youth & Family Services, Inc.
- · Complete progress notes, incident reports, and record vital signs using THERAP ™ according to outcomes and supports of individuals as outlined in their person centered plan.
- · Support individual’s receiving services in planning and structuring activities.
- · Bring your own skills, strengths and interests to supporting individuals’ activities.
- · Support individuals receiving services to exercise their rights and responsibilities in all areas of their lives.

Duties:
- Reading, writing, and carrying out directions
- Charting or taking notes for others to review
- Providing companionship and supportive listening
- Reporting changes in the health or status of the client
- Taking vitals, height, and weight
- Feeding clients
- Maintaining patient’s health equipment
- Responding to emergency situations
- Administering medication
- Transporting client to appointments
- Facilitating community and recreational outings, including transport
- Providing support with hobbies, studies, and social activities
Hours per week:
