Position Title: Business Analyst - Population Health Management
Position Summary:
Coordinates population health initiatives by gathering, analyzing, interpreting, and presenting data to the physician network group and collaborating partners. Helps develop and drive network wide population health improvement strategies through the delivery of population health analytics. He/she will use understanding of current and developing concepts of the value based strategies, Quadruple Aim, and other emerging healthcare reform models to inform methods and solutions. He/she will provide training to advance the skills of department team members with a strong focus on population health improvements.
What the Business Analyst - Population Health Management will need:
- Bachelor’s Degree in Healthcare, Business, or another related field
- Valid Michigan driver's license and proof of auto insurance
- Two years of experience in a healthcare setting. Experience with office management, billing, and/or health plans
What the Business Analyst - Population Health Management will do:
- Responsible for data collection, dashboard maintenance, and management of data delivery to groups, practices, providers and all other stakeholders including Executive Leadership on a monthly basis.
- Develops long-term and short-term strategies to promote effective communication based on unique value propositions of AHN target personas by using AHN communication channels.
- Provides ongoing monitoring and assessment of Affinia Health Network program effectiveness by developing metrics and reporting processes, and analyzing results.
- Maintains and fosters a positive working environment within the department(s) by promoting ongoing interdisciplinary communication and staff participation in active problem solving of clinically integrated network issues.
- Performs quality assurance on all data sources to ensure quality and accuracy as well as usefulness of all AHN tools.
- Recommends revisions of population health systems, processes and workflow to ensure achievement of quadruple aim through critical analyses across the network
- Under the guidance of the Executive Management Group, leads the implementation and project management by becoming a content expert of transformational workflows associated with value based contracts and other initiatives across Affinia Health Network.
- Facilitate and manage workgroups to collect and document technical and clinical business system requirements.
- Plays a key role in the selection and implementation of software solutions related to Population Health Management and trains end users on available tools and management options.
- Gathers, validates, and analyzes data from multiple sources to extract trends and business insights.
- Assists and project manages transformational activities, including participating in APM requirements and initiatives established by our Executive Management Group.
- Implements a measurable, organized, enthusiastic and outcomes-oriented approach to transformational projects and initiatives.
