Social Worker, FT
Mary’s Meadow comprises four separate houses, each of which has accommodations for 10 residents. All rooms are private, with full baths, and each house has a shared living room, dining room, den and kitchen.
This intimate environment makes a real difference for our residents, it encourages interaction and socialization while affording privacy.
The four houses are connected to a central chapel, open to people of all faiths, providing a calm and quiet space for prayer and reflection.
Acts as the primary resident advocate in the facility. Documents as mandated by the DPH regulations and facility policies. Establishes and maintains relationship with other departments and agencies in the community. Provides medically related social services to meet resident needs.
PRIMARY JOB RESPONSIBILITIES AND DUTIES:
Demonstrates, promotes and integrates the Trinity Health Senior Communities Mission and core values by:
1. Maintains knowledge of new admissions and assists in process, i.e., assists residents and family in completing admission packets, completes social history for those residents assigned to caseload.
2. Develops, maintains and utilizes a listing of current community resources/agencies that are useful to residents and their families/significant others. Acts as a liaison between the community and our residents/families.
3. Responsible for the daily interaction with residents and the documentation/or charting of social service interventions a minimum of every three months or per policy and OBRA/Federal/State Guidelines.
4. Assures that any personal care needs are communicated to the family with special attention to clothing needs.
5. Participates as part of the interdisciplinary team in providing interventions to resolve behavior, mood or psychosocial problems and develops and implements plan of care.
6. Participates in the assessment for falls and restraints.
7. Assures that residents are informed of their rights and assist residents in exercising their rights if the resident desires.
8. Facilitates discharge planning based on the goals of the resident and family.
9. Implements and oversees the facilities grievance policy and procedures.
10. Provides groups for residents/family members/significant others as appropriate to their needs.
11. Other duties as assigned.
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:
1. Knowledge of Social Work principles normally acquired with a Bachelor’s of Social Work in order to advocate and preserve the rights of the residents.
2. Massachusetts License in Social Work (LSW) is also required in order to document independently.
3. Knowledge of medical and/or geriatric social work
WORKING CONDITIONS:
1. Must be able to move about the facility independently and be able to document in residents’ charts.
REPORTING RELATIONSHIPS:
1. Reports directly to the Director or Nursing (Quality of Care Coordinator).
