Position Title: HCBS Specialist
Reports to: Senior Branch Manager
Job Description: Responsible for assisting the Senior Branch Manager in overall compliance with Local, State & Federal Laws and Regulations concerning the operation and provision of Home and Community Based Services - HCBS. Participates in developing strategies, goals, policies and regulatory requirements related to HCBS services.
Responsibilities Include:
- Assist in hiring process, interviews, run background checks.
- Assist in the supervision of all HCBS direct care staff.
- Document all phone calls, emails etc. with client, case management and direct care staff.
- Inform and confer with the Senior Branch Manager on all client, personnel and service management needs and problems.
- Responsible for assigning personnel to clients based on client needs and individualized service plan.
- Responsible for scheduling of clients with agency staff.
- Responsible for daily tracking of Electronic Visit Verification usage via HHA Exchange.
- Performs data entry functions as required.
- File/scan documents appropriately and document in HHA Exchange.
- Perform other duties as assigned by the Senior Branch Manager.
- Project a concerned, professional attitude and develop a positive rapport with all staff
- Assure the confidentiality of all client and proprietary information.
- Supervise and direct all HCBS client services.
- Promote, review, and evaluate the quality and appropriateness of client care practices.
- Revise Individualized Service Plan for appropriateness and application.
- Maintain contact with client’s, via telephone, email etc. and client direct care staff.
- Review clients care through on-going communication with staff and coordination with other agencies.
- Receive and act to resolve complaints made by clients and direct care staff, regarding care provided relating to services, personnel or policies and procedures.
- Coordinate and ensure that the client is well serviced through the assignment of services and personal care attendants whose skills and abilities meet their individual needs.
- Assist with training and annual in-service training of all staff.
- Responsible for interviewing, hiring, and orienting all HCBS staff.
- Document phone and in-office conferences with physicians, case managers, clients and direct care staff.
- Maintain high visibility and availability while in the office.
- Assure the confidentiality of all client and proprietary information.
- Other duties as assigned by the Senior Branch Manager.
Job Requirements:
- High School Diploma
- Home Health Experience is a plus
- Organization and Multitasking skills
