Working with assertive outreach workers and rough sleepers within the CBD Why this role matters The Salvation Army Australia’s Homelessness Stream provides high quality, person centred services to people at risk, and or experiencing homelessness. We support adults and families, including accompanying children with accommodation, case management, outreach support, financial assistance, connection and referral to other specialist services. About the role We are seeking a values-driven Outreach Worker who will be working with people experiencing homelessness in The City of Melbourne to secure housing outcomes. Identification of the persons’ immediate housing needs, develop and deliver an effective case management approach for people who are homeless, or at risk of homelessness, including those living in our Magpie Nest Housing Program. Working with housing providers and the private rental market, including head leases, to secure immediate accommodation and/or long-term housing, facilitate access to employment, education, training, health and housing support. This position provides case management support to clients by: Helping access support and resources, advocating for clients as required, where appropriate, working as part of a team with other support workers to achieve best outcomes for clients. You will successfully A high level of quality, effective and integrated services are provided by the Housing Worker to proactively address homelessness. Detailed and holistic case plans are effectively articulated and documented to achieve positive outcomes for clients. Tenant and program advocacy to achieve positive supported housing outcomes. Participants are assessed appropriately for the Magpie Nest Program in accordance with the program and network referral process. Persons who are sleeping rough in The City of Melbourne are engaged with in a positive and proactive manner with rapport established based on trust and respect. Pathways out of homelessness are proactively and positively promoted and implemented to achieve positive outcomes. All tenancy changes are managed effectively, in line with relevant procedures and required timeframes. Rental arrears are kept in line with established key performance indicators. All program applications are assessed expeditiously and properties are optimally managed to ensure effective use. Tenancy agreements, maintenance reports and condition reports are prepared and reviewed regularly with all required reports provided within the required timeframes. Data entry recording systems are implemented in accordance with privacy legislation for monthly collation and audit of data to meet program requirements. This is a fixed term(4 year contract), full-time role, within the Melbourne CBD area, VIC. Salary and conditions are in accordance with the SACS VIC Lvl 4 - Social Wkr CI award. You will have A tertiary qualification in social work and/or related disciplines is desired. A minimum of 3years’ experience within community services (housing and homelessness) is desired A National Police Record Check is required.
