Responsibilities
The Stroke Program Coordinator is fundamental to the development, implementation, and evaluation of the Stroke Program. S/he is responsible for direct patient care and coordination of care and/or case manages for stroke patients based on current clinical practice guidelines across the continuum of care.
As the expert nurse, the Coordinator participates in all stroke alerts to support the delivery of evidence-based acute stroke assessment, care and management. The primary responsibilities are direct patient care which includes nursing process and practice, education, professional development, and customer service to drive quality, improve clinical outcomes, lead post-hospital follow up, and serve as a resource for community outreach.

Using a multidisciplinary approach, s/he will lead, implement, and evaluate process improvement and implemented care pathways. The Stroke Program Coordinator will work collaboratively with all hospital and clinic personnel on relevant performance improvement projects. The Stroke nurse coordinator applies experience and judgment to determine the importance of a situation, sets priorities, and uses abstract thought to evaluate clinical situations. Demonstrates leadership skills and the ability to problem solve on a broad level. Facilitates critical thinking of staff when addressing patient care issues. Provides education and coordination of patient care specifically for the Stroke population. Participates in the development and review of policies, procedures and protocols of new and existing Stroke services. Member of the stroke alert response team – completes initial assessment, supports and facilitates needed interventions, acts as a resource for primary nurse and demonstrates clinical knowledge ensuring care interventions meet professional standards of practice and national stroke guidelines. Conducts chart audits on all stroke patients, ensuring compliance with CSC and PSC core measure and performance standards. Provides feedback as needed and participates in stroke PI initiatives. Attends and participates in all stroke team meetings and hospital-wide committees. Provides specialty education for patients and their families re: stroke risk factors, etiology, risk reduction, and identification of s/s of stroke. Participates with hospital-wide education clinical staff education and community outreach / education activities Position reports directly to the Stroke Program Manager. Demonstrates accountability for own practices as defined by the Nurse Practice Act. Demonstrates investment in improving the overall quality of care in the clinic/unit.
Required Qualifications
- At least 3 years of recent RN experience in a critical care unit,
- Experience working in a neuro specialty setting
- ACLS and BLS Certification
- Valid California Registered Nurse License without restriction
- Graduation from an accredited school of Nursing
- Ability to read, write, speak, and understand the English language.
- Ability to utilize medical terminology appropriately.

Preferred Qualifications
- Pediatric Advanced Life Support (PALS) or obtain within 6 months of hire
- Moderate Sedation certification or obtain within 6 months of hire
- Current NIH Stroke Scale (NIHSS) certification or obtain within 6 months of hire
- Emergency Neurologic Life Support (ENLS)
Special Requirements
- Must be able to wear radiation protection apparel weighing up to 25 lbs for extended periods of time
- Must demonstrate ability in assessment, planning, implementing and evaluating patient care, taking into consideration protective interventions, development (behavioral and physical) and physiological characteristics specific to the patients.
- Has mandatory reporting responsibilities for domestic violence, child abuse and elder/dependent abuse.
- Ability to safely lift 25 pounds and assist in moving patients, and to stand on feet over extended periods.
- Works overtime as defined by the collective bargaining agreements; floats to other areas; and works weekends, holidays and call as required.
