The LPN is a key member of the Northstar Behavioral Health Team who is responsible for delivering quality direct patient care under appropriate clinical supervision.
JOB DUTIES/RESPONSIBILITIES
- Administers patient medications prescribed by physician, ensuring medications are distributed correctly and in a timely manner.
- Assists the dietary staff in identifying patients on special diets and assuring patients’ compliance with dietary orders.
- Carries out general nursing duties to include documentation review, assists with admissions, transfers, and discharges.
- Participates in treatment planning and helps identify patients’ problems, needs, and strengths; participates in implementing treatment plans by monitoring patients’ progress, communicating information regarding patients’ behavior to other members of the treatment team, maintains communication with patient, family members, and referral sources in patients’ interest.
- Oversees general health of residents; provides effective, timely basic nursing care as needed and assigned. Works to ensure effective, timely care referral as needed and assigned to ensure resident health and safety per standard of care.
- Co-facilitates therapeutic group activities and ensures that patients regularly attend and typically participate in program activities.
- Ensures documentation is detailed, concise, clear, and completed in a timely manner.
- Maintains a safe, orderly, and therapeutic physical environment.
- Attends mandatory nurse meetings.
- Obtains vital signs and reports finding to registered nurse.
- Performs chart audits and chart reviews.
- Observe for, and document changes in patient’s appearance, behavior and ability to work toward treatment plan goals.
- Communicate changes in patient’s condition to the Registered Nurse.
- Teach communication, interpersonal and social skills as well as the basic principles of physical health to the patient and significant others, under the supervision of a Registered Nurse.

REQUIREMENTS
Must hold a valid State of Alaska LPN License.
