Responsibilities
Foundations Behavioral Health is currently seeking a full-time Care Navigator to support our Doylestown, PA campus. The Care Navigator is responsible for working with the clinical team and ensuring all discharge planning for patients on unit is completed.
This position is eligible for a sign-on bonus!

Qualifications
QUALIFICATIONS:
- Master’s Degree in Human Services field from accredited college or university along with supervised clinical internship/practicums in area of study.
- Basic computer skills or willingness to learn.
- Two years’ experience with child/adolescent mental health clients in an inpatient or residential setting preferred.
