What are the responsibilities and job description for the Intake LVN - Per-Diem position at UHS?
Come join our team and love what you do! We are currently hiring a per-diem Intake LVN for Fremont Hospital! The Intake LVN is responsible for performing the Initial Medical Screening Assessments for incoming patients, and coordinates with the Medical Staff in directing the patient towards an appropriate level of care.
Fremont Hospital is a fully accredited, 148-bed private inpatient psychiatric hospital providing quality care for children through adults. The facility provides a full assortment of care which includes assessments, inpatient psychiatric hospitalization, partial-hospitalization, intensive outpatient psychiatric treatment as well as chemical dependency outpatient care.

Qualifications
Education: AA/AS Degree or Bachelor’s Level Degree in social work, psychology or related mental health field preferred, PhD and PsyD candidates in terminal degree programs with Masters equivalent or a degree from an accredited program in nursing preferred. PhD and PsyD candidates in terminal degree programs with Masters equivalent or a degree from an accredited program in nursing preferred.
Experience: A minimum of one (1) year direct clinical experience in a psychiatric or mental health setting, with direct experience in patient assessment, family motivation, treatment planning, and communication with external review organizations.
Licensure: LVN Licensure Required
Additional Requirement: CPR with AED (CPR/AED) certification required.
