Havenwyck Hospital is seeking a dynamic and talented Outpatient Navigator to join our team of compassionate, dedicated professionals.
The Outpatient Navigator is responsible for the overall internal growth of the outpatient programs. The Navigator identifies, educates and assists in facilitating individuals from the inpatient unit to the outpatient programs. The Outpatient Navigator also works to improve the overall patient flow and access to outpatient care.
Outpatient Navigator Duties include but are not limited to:
- Provides education and support to staff, physicians, patients and families on the hospital’s outpatient services programs.
- Identifies potential patients from the inpatient units that can transfer to the PHP/IOP and help to facilitate that process.
- Reviews data and prepare reports related to the outpatient services programs.
- Assists inpatient social workers/case managers with discharge planning.
- Collaborates with the Business Development team on outpatient services collateral.
- Assists in the development and achievement of departmental goals and objectives within the overall strategic plan of the hospital/corporation.
- Assists in the daily functions of the outpatient programs as necessary.
- Meets all competency requirements annually per department standards, which will include age specific-growth and development competencies.
- Actively promotes customer service throughout the hospital by practicing team building and developing partnerships with other departments.
Qualifications
- Bachelor’s degree in a social service field required. Master of Social Work degree from an accredited School of Social Work and/or a MA degree preferred.
- Minimum of 1 - 2 years of experience in a mental health setting required.
- Experience in an inpatient psychiatric unit preferred.
- Employees must be in good standing (employment/corrective action history, performance evaluations, etc.) to be considered for an internal transfer.
