What are the responsibilities and job description for the Case Manager position at Unified Community Services?
Unified Community Services has a full-time Social Worker/Children’s Programs Case Manager position available based in our Dodgeville and/or Lancaster office. Responsibilities include coordination of services for children with special needs in a variety of programs such as Birth to 3 and Children’s Long-Term Support. Responsible for coordinating services for the family involved in receiving services and enabling the eligible child to receive services.
These positions require a bachelor’s degree in social work or human service related field and a minimum of 1-year experience working with families and children with special needs. Social Worker certification or eligibility a requirement. Requires a valid driver’s license, reliable transportation, and completion of caregiver background check.
