Description
Under the direction of the Employment Connections Manager, the Job Coach is responsible to help clients obtain and maintain employment.

DIRECT SUPPORT:
Assists individuals in defining job goals, learning job tasks, and problem-solving various barriers to employment. Adapts tasks and/or methods of work, to meet the individual's abilities while maintaining employer set standards. Develops on the job "natural supports" which will enable the individual to be as independent of staff intervention, as possible. Acts as a role model whenever in the presence of an individual. Serves as a liaison between the individual and work site when necessary. Helps the individual increase skills in all areas related to employment. Provides case management services to individuals including assessment, advocacy and monitoring. Performs med administration, as assigned. Facilitates community integration. Provides transportation for individuals when applicable. Attends all training's, as assigned.
RECORD KEEPING:
Keeps accurate documentation about individual progress. Maintains formalized employment plans and goals for individuals according to their interests and abilities, which corresponds to the Individual Service Agreement. Completes incident reports within the required time period. Completes monthly summaries in a timely fashion.
NETWORKING:
Works as a team member of Employment Connections, Developmental Services Division, and UCS. Communicates appropriate information to other staff, employers, family, guardian, case manager and QDDP. Serves as advocate and/or liaison on behalf of individual, program and/or agency with other agencies and professionals in order to provide quality services to individual. Serves as ambassador for the Employment Connections Program to the community.
