Requirements
The Shared Living Case Manager develops, implements, coordinates, and monitors the provision of services to individuals with developmental disabilities who reside in a Shared Living Home, as assigned to them. This individual will assist clients in identifying their strengths, needs, and goals while working to provide resources to reach these objectives. This Shared Living Case Manager will join a supportive team participating in fulfilling work.
CASE MANAGEMENT SERVICES: Oversee the person-centered program planning through the supervision, development, and management of the ISA process. Complete necessary documentation in a timely fashion. Assist clients in the identification of strengths, needs, and goals. Train staff (possibly in interaction strategies), as requested, in methods and strategies, and monitors for implementation and effectiveness. Interact with assigned clients on a regular basis. Review ISA expectations, objectives, and data collection systems to ensure smooth, consistent implementation and that services are provided as outlined in the ISA. Ensure that all regulations are adhered to. Performs medication administration, as assigned. Participates as a member of the DS Emergency On-Call System.
SUPERVISION OF HOME: Participate in the development of a new home for assigned clients. Provide training, oversight, and support to the assigned contracted home providers. Ensure adherence to regulations and the contract. Review and process monthly documentation. Visit the home minimally one time per month, more often as needed.
PROGRAM COORDINATION/PROGRAM DEVELOPMENT: Ensure that all waiver services are scheduled and being received by each individual. In conjunction with the ISA team members, develops individualized services for each individual. Serves as liaison between agency personnel and others when developing services. Ensures goals, objectives and record keeping systems are developed accurately, written, and in place in a timely fashion. Evaluate individual programs for effectiveness, recommending changes to the QDDP, when needed. Ensure program are consistently carried out.
CLIENT ADVOCACY AND NETWORKING: Respond to requests for information and/or technical assistance, as appropriate. Communicates with staff or other agency programs and/or other human service agencies to obtain/or provide all relevant individual information. Refer individuals to appropriate services and resources. Maintain contact with other service providers. Develop resources and essential individual support services needed. Serve as an advocate on behalf of individual, Shared Living providers, Developmental Services and UCS. Effectively represent the agency to the general public and to referral sources, explaining agency functions, operations and philosophy. Maximize the agency/division's reputation and cultivate support for agency services. Function as a representative of the agency, as assigned. Serve on an agency or division committee, as assigned.
