Job Summary:
Has the responsibility for providing direct and indirect nursing care to PACE participants. Works under the direct and indirect supervision of the Nurse Manager or designee but is independent in the application of advanced nursing knowledge and skills. Manages complex clinical situations for which he/she is responsible.
Primary Responsibilities:
- Collects patient data and completes required forms with appropriate responses according to PACE standards.
- Identifies participant’s overt problems/needs and sets priorities.
- Identifies problems requiring further referral and/or follow-up.
- Interprets and records latest diagnostic results; performs advanced nursing assessment using critical thinking skills.
- Participates as a member of the Interdisciplinary Team. Attends staff meetings and takes part in participant care planning as per Nurse Manager.
- Completes in-home assessments of prospective participants as assigned
- Completes initial, semi-annual and annual assessments of all participants as assigned, prior to the scheduled team meeting
- Develops a plan of care based on nursing process and which incorporates the plans of other disciplines and continuing care needs.
- Includes the patient/family in developing or revising the plan of care.
- Makes referrals to interdisciplinary support services.
- Communicates participant changes to team members.
- Assesses, using the nursing process, the home care and clinical needs of a frail elderly population, and identifies and develops specific plans of care.
- Provides correct treatments/procedures and other care as prescribed and according to participant care standards including but not limited to IV therapy, tube feeding, and wound care.
- Provides home visits for scheduled nursing care
- Provides medication management, including gathering of initial medication list at time of enrollment, and communicates needs and follows up with pharmacy. Provides medication education to participant and/or family as needed
- Assists with Initial and periodic assessments to be conducted minimally every 6 months and must be completed prior to the scheduled team meeting.
- Management of 24-hour care delivery in the home and the implementation of all home care services which include nursing, PT, OT, ST and personal care services to ensure that quality services are provided to meet participant needs.
- Assists with monthly CENA in-home supervision visits to ensure in-home care plan is being followed.
- Assists with DME and supplies ordering and coordination within the home
- Coordinates and communicates with all nursing care for participants in nursing homes/Adult Foster Care Homes and Assisted Living Facilities according to program standards. Includes interaction with other facility staff, this to include but not limited to visits for wellness, labs, vaccines, etc.
- Demonstrates active participation in QI processes.
- Attends staff meetings and takes part in participant care planning.
- Documents services in the Electronic Medical Record.
- Communicates any changes in participant condition to team members.
- Ensures participants rights to fair and equitable treatment, self-determination, individuality, privacy, property and civil rights, including the right to a prompt response to complaints.
- Participates in the development and revision of the enrollee’s plan of care as a member of the interdisciplinary team.
- Ensures appropriate administration of medications at home.
- Assigned duties in the clinic when not in the field.
- Participants in the nursing on-call rotation
- Adheres to work flows for clinic operations as established by the Nurse Manager and/or the Medical Director.
- Attends clinic meetings and huddles as assigned by Nurse Manager
- Using nursing process, delegates nursing care to appropriate personnel.
- Integrates cost effective measures into nursing practice.
- Recognizes unit problems and takes responsibility for documenting same.
- Demonstrates active participation in QI processes.
- Complies with policies addressing safe working conditions. Reports unsafe working conditions
- Recognizes inappropriate and/or ineffective patient care management, resolves issue/problem and completes written reports.
- Fulfills on call responsibilities.
- Must possess knowledge and skills necessary to treat patients.
- Only act within the scope of his or her authority to practice.
- Meet a standardized set of competencies for the specific position description established by Thome PACE and approved by CMS before working independently.

- High School Diploma/GED
- Graduate of an accredited School of Nursing.
- Licensed Registered Nurse in the State of Michigan.
- Must be CPR certified
- Minimum of one (1) year working with the frail and elderly.
- All employment offers are contingent upon successful completion of a pre-employment criminal background check, verification of professional licensure, if applicable, and TB Screen at time of hire. In addition, proof of COVID-19 vaccination or approved exemption is required.
- Immunizations and vaccines must be current or medical documentation from a medical provider to request a waiver.
- Must have three (3) years’ experience as a practicing nurse in a health-related area
