What are the responsibilities and job description for the LPN/RN position at United Woundcare Institute?
The United Care Team under AWI is a highly dedicated multi-disciplinary team of providers specialized in wound care. We provide comprehensive end-to-end treatment of sores or wounds that have not significantly improved from conventional treatments.
a. Contact patients to schedule date/time for provider visits
b. Perform patient evaluations and submit it to the Medical Director (wound pictures and wound measurements).
c. Responsible for drawing blood from patients
d. Confirm you have the proper supplies for patient visits for provider, order supplies, and making sure bag is properly stocked at all times
e. Assisting provider in procedures with direction from provider
f. Take notes from provider and input into EMR and other related documentation
g. Traveling to patient homes
h. Prefer Certified and Registered Nurse
i. Report to Director of Operations, Michigan
1. Assist in implementation of United Woundcare Institute’s (“UWI”) clinical and operational protocols.
2. Assist in implementation of the advanced clinical protocols, including but not limited to advanced modalities such as skin substitutes, misting, negative pressure therapy, etc.
3. Assist in mentoring other staff and co-workers to implement The Institute’s protocols.
4. Collaborate with referral sources to ensure delivery of outstanding care.
5. Represent UWI to various market participants, such as nursing facilities, etc.
6. Provide effective, efficient, professional, and timely communication with patient, family, coworkers, management and care partners.
7. Perform patient evaluations in coordination with regional team.
8. Perform tasks assigned by other team members such as medical directors, business development, managers, and others.
9. Schedule, organize and attend necessary meetings on time and participate in program development and performance improvement.
10. Assist with patient documentation and provider scribing to complete encounter notes per policy timeframes.
11. Document patient communications in EMR system per policy.
12. All other tasks and action items as requested and approved by leadership.
Considerations:
1. Position is a mobile position with higher-than-average travel time.
2. Position involves direct patient care, potentially heavy lifting, and clinical risks associated with healthcare services.
3. Position does allow work-from-home status per leadership and requirements (subject to change at any time).
4. Wound care is performed at the patient’s ‘home’, residence, and regional facilities.
Salary : $57,900 - $73,300
