OptumCare Washington is looking for full time Advanced Practice Clinicians (APC, either a nurse practitioner/physician assistant) to provide clinical care across a variety of settings and address the gaps in care that are often missed in traditional clinic visits.
Join our team in providing integrated, supportive care to a complex aging population with a goal of reduced suffered, increased patient and family understanding, and optimized care in SNF and long-term care settings.
The Everett Clinic is looking for a growth-minded APC with experience in SNF/Long-term care work, hospital medicine, geriatrics or palliative medicine to join our Geriatric Extended Care (GEC) team. Our team partners with Skilled Nursing Facilities (SNFs) in Snohomish County to round on our medically complex patients.
Primary Responsibilities:
- Rounding on patients in the SNF and long-term care (LTC) setting.
- APC is responsible for covering 1 to 2 SNFs as the main continuity provider at SNF/LTC facility.
- Patient average encounters/visits per day will range from 6-10, typically depending on patient complexity, acuity, and number of SNF/LTC facilities covered by APC. Average daily skilled census between 1-2 SNFs ranges 6-13.
- 0.8-1.0 FTE =4-5 days a week SNF/LTC coverage, M-F, typically 8-hour work days.
- Attend and actively engage weekly in IDTs at SNF, departmental, and multi-specialty meetings.
- Work collaboratively with Extended Care physicians and the patient’s primary care team to prioritize patient goals and provide high quality/value-based care.

Required Qualifications:
Open to licensed APC with ideal education or work experience in SNF/long-term. Potentially opened to candidates with hospital work experience or primary care with a geriatric background.
- Current Washington state license or ability to obtain prior to employment.
- Nurse Practitioner Certification (NP-C) or PA (physician assistant) certification.
- Excellent communication skills, high patient satisfaction, and demonstrated experience promoting teamwork

Washington Residents Only: The salary range for Washington residents is $110,040 to $139,000.
