This position is full - time, Monday - Friday. Employees are required to work our normal business hours of 8:00 AM - 5:00 PM. It may be necessary, given the business need, to work occasional overtime or weekends. Our office is located at 26677 W. TWELVE MILE, SOUTHFIELD, MICHIGAN, 48034. Training will be conducted virtually from your home.

Primary Responsibilities:
Outreach Support Specialist will be responsible for supporting the Local Outreach team in follow up and outreach of non-engaged patients
Will perform administrative tasks creating and / or pulling reports to support outreach to community providers for market leaders
Make outbound and inbound calls to confirm patients for in-home clinical visits with a Landmark Health provider
Meet quality and production metrics to provide the highest level of customer service on each interaction
Responsible for learning the outreach model and adapting it to constantly changing patient interactions to meet business and patient needs
Must be goal oriented and willing to adjust to a fast paced and evolving environment

Required Qualifications:
High School Diploma / GED (or higher)
Proficiency in using Microsoft Excel (entering data, updating spreadsheets, primarily data entry purposes) and Microsoft Outlook (manage emails and calendars)
Any customer service experience in healthcare setting, making phone calls and performing basic data entry tasks (i.e., medical front office environment, patient scheduling, appointment reminders, etc.)
Ability to work any 8-hour shift between the hours of 8:00 AM - 5:00 PM from Monday - Friday

Preferred Qualifications:
Familiarity with EMR system (electronic medical record)
Experience and knowledge of HIPAA guidelines and regulations
Experience with making inbound OR outbound call, such as call center
Experience with proper call routing to resolve patient issues
Telecommuting Requirements:
Reside within South Detroit, Michigan area (including flexibility to visit the office for team meetings and / OR events)
Required to have a dedicated work area established that is separated from other living areas and provides information privacy
Ability to keep all company sensitive documents secure (if applicable)
Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service

Develop a thorough understanding of the Landmark clinical program
Engage patients in the Landmark Health Program cohort
Establish rapport with qualifying patients
Maintain and ensure the confidentiality of all Protected Health Information
