What are the responsibilities and job description for the Clinical Pharmacist Specialist (Surgery-Liver Transplant) position at University Hospital?
The primary purpose of the Clinical Pharmacy Specialist position is to optimize patient medication for all ages and populations served by University Hospital by providing comprehensive pharmacy services. Works collaboratively with healthcare team to accomplish departmental golas. Provide guidance that improves both the use of medications by patients and the medication use process in ways that enhance patient's health related quality of life and patient outcomes.
Candidate must possess a Pharm. D., Master's Degree of Pharmacy, or Bachelor's Degree from an accredited school of Pharmacy with Board Certification in a specialized area. Pharmacy Practice Residency or equivalent is preferred. A minimum of one (1) year of hospital experience with enhanced clinical responsibilities required.
