Job Summary:
Nature of Work:
The University of Colorado College of Nursing, is seeking an RN for Case Management for a full-time (1.0 FTE), benefitted position. This position will split time between the Family Health Clinic, in Denver, CO (80236) and Belleview Point Clinic in Aurora, CO (80015). This RN should be team oriented and willing to uphold the Sheridan Health Services (SHS) mission by assuring that patients receive health care that is competent, caring, cost-effective, and met with a high degree of satisfaction. Sheridan Health Services is a Patient-Centered Medical Home, and as such we uphold a model of care that places the patient at the center, incorporating the values and beliefs of the patient in all decision-making. This position is responsible for providing patient care coordination including helping patients connect with specialty care, providing clinical care for adults, help families access available community services, as well as triaging patient phone calls throughout the day.
Clinic Description:
SHS is a multi-site, nurse-managed FQHC, funded through the Health Resources and Services Administration Bureau of Primary Healthcare; and is a collaborative effort between the College of Nursing and the Sheridan School District. There are three convenient locations to serve community health needs primarily for residents of Arapahoe, Denver, Adams, and Jefferson counties. The CU Youth Health Clinic is in the Sheridan School District Administration building in Englewood, CO. The CU Belleview Point Health Clinic is located in southwest Aurora and the CU Family Health Clinic is located in southwest Denver on the Fort Logan campus and provides services to adults and children, 65% of whom identify as Latinx, and 42% of whom are best served in another language, with Spanish being the predominant second language. Services offered include primary care across the lifespan; prenatal and postpartum, gynecology, dentistry, pharmacy, behavioral health, and outpatient substance abuse treatment. SHS is a Patient-Centered Medical Home, and as such, we uphold a model of care that places the patient at the center, incorporating the values and beliefs of the patient in all decision-making. We extend the reach of the clinic through public health outreach where nursing students provide community outreach, health education, and home visitation services.
Depending on qualifications and department needs, appointments may be made at the Entry Professional, Intermediate Professional, Senior Professional or Principal Professional level.
Examples of Work Performed at All Levels:
Care Coordination
- Case management and care
- Coordination across service lines for mutual patients (ie Adult & Pediatric Primary Care, Behavioral Health and Psychiatry)
- Work closely with Adult Primary Care teams at FHC and BVP to assist with care coordination and patient care needs
- Coordination of care with community partners
- Actively searches for beneficial programs and services for SHS patients
- Maintains informative brochures and educational materials
- Meets with patients to assess social and medical needs
- Works with other SHS providers to provide comprehensive and integrated care
Phone Triage
- Speaks with patients that call the clinic with health questions and provides guidance
- Works with providers to address patient care needs over the phone.
- Calls patients with testing results and health education
Health Coaching
- Encourages patients to set positive lifestyle changing goals to improve overall health and well-being
- Follows up with established patients on a regular basis and helps them to assess goal progress and recognize achievements
Student/Medical Assistant Support and Training
- Assists with orientation for new students, faculty, and staff to the clinic
- Serves as a resource for MAs and support staff to assist with patient care
- May exercise supervision of Medical Assistants and referral coordinator if appropriate
Additional Duties
- Focuses on population-based health to improve preventive care/immunizations rates and chronic disease outcomes
- Ensures infrastructure for implementing, tracking and reporting progress on quality and grant-related activities and measures
- Documents patient care using the electronic medical record system
- Takes an active role in promoting and building the integrated health team
- Participate in community outreach events. (Weekend availability for participation as needed)
- Remain credentialed ensure credentialing materials stay current
- Provides coverage during absence of other RN staff at both clinic sites
- Takes on assigned projects as needed

- Current RN licensure in good standing (any state)

Intermediate Professional

- 1 year of professional level RN experience

Preferred Qualifications (All Levels):

- Experience providing care coordination in a Federally Qualified Health Center, a public health setting, and/or to low-income families
- Experience in program development and implementation
- Case Management certification and/or 2 years or more experience with case management
- Experience managing personnel
