Position Summary | The Patient Navigator at the University of Georgia Health Center connects patients with campus and community resources to enhance student well-being and reduce health disparities. This position reports to both the Director of Student Care and Outreach and the UHC Director of Nursing and serves as a member of the Campus-wide Case Management team. |

Health Equity Treatment Plans: Collaborates with UHC clinicians and support staff to design and execute health equity treatment plans, to include follow-up, for patient identified social related health needs. Collects, analyzes, and reports data from health equity treatment plans and follow ups in order to determine student needs and develop interventions designed to meet those needs. |

Case Management: Serve as point of contact for inquiries to SCO; address and respond to referrals made to the SCO, including direct outreach to students, families, faculty, and staff. Facilitate hardship withdrawal process on behalf of students of concern. Serve as a member of the Campus-wide Case Management Team and support the Director of SCO in designing interventions for students discussed in those meetings. Consult with staff to determine resources Assist with management and oversight of SCO records within Advocate Simplicity electronic database. Ensure that records and data are maintained in accordance with University policies and federal and state regulations. |
