Job Summary:
The Bilingual Community Health Worker is responsible for exemplifying the UTHSCT’s Mission, Vision, and Values, providing outstanding customer service. The bilingual Community Health Worker (CHW) will function as a member of a multidisciplinary Texas Home Visiting team that recruits and serves low-income, first-time pregnant woman to support healthy pregnancy, births, early childhood. Primary responsibilities include outreach in Smith and Henderson Counties to Spanish-speaking families. CHW facilitates referrals for health improvement, health promotion, and support. CHW collaborates with other partners in the community to promote family health. This position works as a team member and reports directly to the Manager assigned. Performs other related duties as required/assigned.
Major Responsibilities / Duties / Critical Tasks:
- Participate in outreach activities in Smith and Henderson Counties.
- Assess and assist individuals for potential eligibility for grants, programs, and/or community resources.
- Assist clients in completion of forms/applications required to receive needed services or as required by referral agencies.
- Provide families with referrals to external community resources (such as food pantries) as needed.
- Complete timely documentation and required reporting for all services provided in the PSC/CHW template.
- Comply with hospital and legal requirements by fostering practices that adhere to the hospitals and nursing division’s philosophy goals, and standards of care.
- Respect patients by recognizing their rights, privacy, and maintaining confidentiality.
- Maintain quality service by establishing and enforcing organization standards.
- Cultivate relationships with organizations that may make referrals.
- Receive referrals from other agencies, log the referrals, and follow up to explain the services and facilitate enrollment.
- Contribute to team effort by accomplishing related results as needed.
- Collaborate with Texas Home Visiting nursing staff to promote population health with Spanish-speaking families.
- Maintain professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; benchmarking state-of-the-art practices; participating in professional societies.
- Demonstrate a high level of understanding of HIPAA.
- Handle all phone calls, emails, and in person interactions cordially and professionally.
- Maintain CHW certification.
- Other related duties as assigned.

Required Education / Experience:
- High School diploma or equivalent.
- Two (2) years of health/clinical experience or equivalent health-related education.
- Current Community Health Worker certification by the Texas Department of State Health Services or obtained within 3 months of hire. Must maintain current CHW certification.
- CPR certification preferred or obtained within 3 months of hire.
- Must maintain current CPR certification.
- Knowledge of and willingness to learn about services for families in Smith and Henderson Counties.
- Bilingual in English and Spanish.
