Job Summary: Under the direction of a licensed social worker (Social Worker II), provides biopsychosocial assessments and treatment interventions through the use of structured educational and therapeutic groups and individual treatment formats. Provides case management and care coordination to facilitate access to appropriate services and supports and adherence to treatment plans. Job Description: Essential Duties and Responsibilities: Provide patient biopsychosocial intake assessments, diagnoses Develop individual treatment plans or interventions with patients; modify treatment plans to comply with changes in patients’ status Collaborate with other professionals to evaluate patients' health conditions and needs; review treatment plans with applicable health care providers Monitor, evaluate and record patient progress in the electronic medical record Facilitate referrals for assistance with medications, finances, housing, education, transportation, food and clothing, legal issues (guardianship, durable power of attorney, etc.) Liaise with community agencies to advocate for patients’ needs and community resources Establish and maintain productive working relationships with people inside and outside the department and/or practice Supervisory Responsibilities: None Additional Responsibilities: Other duties as may be assigned Qualifications: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Education and/or Experience: Bachelor’s degree or Master's degree from an educational program accredited by the Council on Social Work Education Michigan Limited BSW or Limited LMSW License 2 years post-graduate clinical experience with 1 year in a managed care population Knowledge of principles, methods and procedures for diagnosis, treatment and rehabilitation of physical and mental dysfunctions Skills and Abilities Required: Skills in medication assessment Skills in interviewing and eliciting information and cooperation from patients Good written and verbal communication skills to convey and receive information effectively (face-to-face, telephone and electronic) Good analytical and judgment skills Ability to use computer software programs and systems Ability to maintain confidentiality regarding patients, personnel and/or financial data Excellent organizational skills to work independently, prioritize, handle multiple tasks and manage time Safety Requirements: Compliance with departmental Health and Safety policies and procedures Mandatory TB test annually (based on Department guidelines) Physical Demands: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
