POSITION DESCRIPTION
General Summary
UMIT Health System is currently operating a clinic, pharmacy, and Lab in King William County and is expanding into home healthcare. UMIT Home Health is providing in-home skilled services (Nursing, PT, and OT) and in-home personal care services. Providing high quality healthcare to our patients is important to us, and we are looking for individuals that strive to deliver high quality healthcare and positive outcome for patients.
Position
Intake Coordinator
Responsible for registering patients from referring facilities, gathering patient information, and placing the patient based on services needed. Work with internal staff and external partners to schedule initial evaluation for skilled and aide services. Properly hand-off case management once patient is admitted to the appropriate Care Coordinator.
Primary Duties and Responsibilities:
· Responsible for answering incoming calls from referring facilities and patients.
· Coordinate all admissions of patients in a fast-paced environment.
· Review incoming referrals to ensure proper placement
· Obtain and verify medical insurance coverage and benefits for each patient.
· Schedule initial patient evaluation with internal or external resources.
· Provide effective communication to all stakeholders – patients, staff and referral sources.
· Introduce Care Coordinator once initial evaluation is complete.
· Perform clerical work, such as filing, copying, and faxing.
Knowledge, Skills and Ability Qualifications:
· High school diploma or GED.
· A minimum of two years' experience in patient intake or medical office administration.
· Strong affinity for customer service.
· Working knowledge of office management and filing systems.
· Excellent organizational and time-management skills.
· Great communication and interpersonal skills.
· Proficient computer literacy.
· Ability to work in a fast-paced environment.
