Responsibilities
Reporting to the Director of Social Services/Social Services Supervisor, the Senior Case Manager provides and documents clinical and case management services for clients. He/She coordinates medical and mental health assessments, medication management, and housing applications.
- Provides information and guidance to case managers, client services and community resources.
- Assigns caseload to case managers.
- Provides support and guidance to case managers in the execution of all housing related tasks including, but not limited to preparation, coordination, and processing of 2010E applications, General Population applications, housing searches, review of housing applications prior to submission to DHS, and housing interviews.
- Provides comprehensive case management services for assigned clients including but not limited to entitlements, housing, employment/education, vocational training, health, mental health treatment, and substance abuse services, and social and community integration.
- Provides counseling and support to clients on an on-going basis including, but not limited to crisis intervention, training and support on daily living skills, assistance in problem solving, conflict resolution, and advocacy.
- Conducts client intakes, assesses client needs, develops service plans and/or service referrals to achieve self-sufficiency.
- Conducts field work including, but not limited to home visits, outreach, and escorting clients to appointments.
- Monitors client medications and coordinates all required medical and mental health appointments for clients.
- Generates program reports and oversees accuracy of client information in client database system(s).
- Liaises with funding agency, community organizations, and housing providers regarding all housing related services.
- Facilitates client groups and staff training as required.
- Supervises all incident reporting and other client activities to ensure proper documentation and data entry.
- Maintains and submits all necessary documents and reports as required by Urban Pathways, regulatory and funding agency policies, procedures, and regulations.
- Attends staff meetings, trainings, and supervisory sessions, as required.
- Develops and maintains relationships with community services and resources.
- Performs all other duties as assigned.
Qualifications
- Bachelor’s degree required. Bachelor’s degree in Social Work or related field preferred.
- Minimum two years of clinical, individual or group counseling experience preferred.
- Experience with homeless, mentally-ill and substance abuse population preferred.
- Knowledge of NYC housing subsidy programs
- Strong computer skills and knowledge of Microsoft Office (e.g. Word, Outlook, Excel, etc.)
- Strong written and verbal communication skills.
- Strong team building and coaching skills.
- Strong problem-solving skills and resourcefulness.
- Strong interpersonal skills and ability to work independently and in a team.
- Foreign language a plus.
