Duties

Patient Screening

  • Duties include performing observation-based screening, collecting patient information in collaboration with licensed personnel. This may involve obtaining patient health history, chief complaints, medication history, and information about health-related or high-risk conditions that may impact the patient's health status. The incumbent will complete approved templates and tools, for example the MiniCog screening established by the agency.
Mental Health Services
  • The incumbent assists in behavioral/psychiatric situations within the critical care setting. This can include assisting licensed personnel with crisis intervention, assisting with observation and documentation of aggressive or suicidal psychiatric patients by providing one-to-one observations and applying Prevention and Management of Disruptive Behavior (PMDB) techniques. The incumbent performs duties focused on communication techniques required to complete the questionnaires and templates approved for use in the intensive care unit, stepdown unit and/or other critical care venues, including active listening, restating, and mirroring. The incumbent will maintain a safe and therapeutic environment for the patient while promoting a sense of worth and dignity under the supervision of licensed personnel.
Clinical Tasks
  • The incumbent assists licensed personnel with the delivery of care to patients with complex, specialized, and potentially life-threatening situations. This may include setting up equipment and passing instruments in the event of defibrillation, cardioversions, insertion of chest drainage tubes, insertion of thoracostomy tubes, rapid sequence intubation, or initiation of mechanical ventilation to restore normal physiological function or prevent deterioration of patient's condition. Clinical duties include but are not limited to:
    • Diagnostic and POC (point of care) studies. Examples: I-STAT, glucometer, bedside ultrasound, vital signs, EKGs.
    • Equipment and procedural set up. Examples: chest tube insertion, assisting with placement of central venous access devices (CVAD).
    • Observations and interventions. Examples: oxygen therapy, thermal regulation, ambulation, oral/trach care.
    • Wound care. Examples: vacuum-assisted closure (wound vac), dressings, topical ointments, wound measurements.
    • Specimen collection. Examples: venous blood samples, urine, sputum, stool and wound samples and cultures.
    • Preparation and transfer to the lab for processing: Examples: labeling, packaging (ice, heat, light protection).
    • Assisting with patient exams and screening. Examples: orthostatic vital signs, 6-minute walk assessment, CAM and fall risk.
    • Patient transport as secondary assistant. Examples: with or without cardiac monitoring and/or ventilation to and from diagnostic tests and/or transfers of Patients.
    • Cardiovascular monitoring. Examples: central station or bedside cardiac monitoring.
    • Thoroughly and appropriately documents actions, interventions and procedures in the electronic health record (EHR).
CTs may perform tasks after proper education, training and competency, demonstrating knowledge, skills and abilities.

Emergency Response
  • The incumbent may be asked to respond to medical emergencies on the facility grounds, and sometimes encounters situations which may be life threatening as part of the Rapid Response Team (RRT), Code Blue, or medical transport team. The incumbent appropriately implements emergency care in accordance with established protocols and policies. Works in concert with other members of these teams to determine the nature, extent, and seriousness of the patient's condition including the search for medical alert symbols and medications. ICTs will obtain and maintain certification in basic life support (BLS), and other certifications as pertinent to their local facility.
Medication Administration
  • Medication administration will comply with national directives and local policies utilizing Bar Code Medication Administration (BCMA) as required and whenever applicable. Administration methods may include intramuscular, oral, topical, eye drops, ear drops, intranasal, intraosseous, rectal suppositories, normal saline IV push, local anesthetics, nebulized medications, subcutaneous, and intradermal. To ensure safe medication preparation and administration the incumbent will perform the "seven rights" (right patient, right drug, right dose, right time, right route, right reason, right documentation). The incumbent will screen patients for contraindications and medication allergies documented or reported and follows all national and local pharmacy guidelines for disposal of medications.

Work Schedule: 7:30 pm - 8:00 am & 12:00 am - 8:00 am. (6) 12 hr shifts & (1) 8 hr shift biweekly

Qualifications

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • English Language Proficiency: In accordance with 38 U.S.C. 7402(d), No person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. You must be proficient in basic written and spoken English in order to meet the requirements of this position
  • Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-7 position you must have served 52 weeks at the GS-6. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment.

In addition to meeting the basic requirements, to qualify at the GS-7 level, applicants must meet the following requirement..

You MUST possess at least one year of specialized experience that equipped you with the particular knowledge, skills and abilities to perform successfully the duties of Intermediate Care Technician position, and that is typically in or related to the work of this position.To be creditable, this experience must have been equivalent to at least the GS-6 grade level in the Federal Service. Qualifying experience includes;

  • Telemetry Experience/training with rhythm recognition.
  • Knowledge of telemetry equipment and telemetry terminology.
  • Critical thinking skills.
  • Skills to meet emergency and routine situations.
  • Prioritization of care/tasks.
  • Recognizes life threatening situations.
  • Working knowledge of diagnostic signs.

SUBSTITUTION OF EDUCATION FOR EXPERIENCE: Successfully completed one (1) year of graduate education or internship directly related to the position at the next lower grade level. This education must have been obtained in an accredited college or university. One year of full-time graduate education is considered to be the number of credit hours that the school attended has determined to represent one (1) year of full-time study. If that number cannot be obtained from the school, 18 semester hours will be considered an academic year of graduate study. Part-time graduate education is creditable in accordance with its relationship to a year of full-time study at the school attended. (COPY OF OFFICIAL OR UNOFFICIAL TRANSCRIPTS REQUIRED)

COMBINATION OF EDUCATION AND EXPERIENCE: Equivalent combinations of creditable specialized experience and education as described above to meet the total experience requirements for the position. This education must have been obtained in an accredited college or university. Only education beyond a one-half year (e.g. 6 months; 9 semester hours) of graduate level education may be used to qualify applicants for the GS-07 level. NOTE: If using education combined with specialized experience to qualify, a copy of your transcript is required.

Preferred Qualification: Military medic training.

Physical Requirements: Heavy lifting. 45 lbs. and over, Moderate carrying. 15-44 lbs., Straight pulling , Pushing , use of fingers, both hands required, walking , standing , repeated bending, both legs required, near vision correctable at 13" to 16" to Jaeger 1 to 4, far vision correctable in one eye to 20/50 and tp 20/100 in the other, ability to distinguish basic colors, hearing (aid permitted).
